Document General Information of Patients

You can document a patient's general information on the Intake template. When someone provides the patient details, you can document the historian details in the Historian field. When someone interprets the patient’s communication, you can select the Interpreter Details link to document the details of the interpreter. You can select Configurable Questionnaires link to document the screening details of the patient. You can select the Transitioning into Care link to document the summary of care record information for a patient. You can select the Enable HCC for this patient check box to hide or show the HCC links, badges, and grid columns on the templates at the patient level. You can select the Family planning encounter and Disability check boxes if an encounter is a family planning encounter and describes any disability. Viewing Patient Insights

If your patient in the Primary Care, Federally Qualified Health Centers (FQHC), or Behavioral Health specialty, synchronized with Population Health System (PHS) has clinical gaps in care, you can assess and resolve the Hierarchical Condition Categories (HCC) diagnoses in the Patient Insights section in Patient Chart. You can provide the patient the necessary care and help your practice obtain reimbursement depending on the patient's insurance. The gaps count of upcoming and overdue items, and the Adjusted Clinical Groups (ACG) risk score for the patient appear on the header. You can select the Patient Insights section to view the following details: Risk score overview In Risk Score, the ACG, Atherosclerotic Cardiovascular Disease (ASCVD), and HCC scores appear for patients having clinic visits in the last twelve months. Documenting Assessment Details

You can add or update a patient's assessment details in the Assessments tab. You can update the ICD-10 coding and 5010 standards using the patient's assessment. You can select the assessment details in the grid and submit the first 12 diagnoses for claim processing. You can add or update an assessment in the following sections: Billing Diagnosis History Clinical Problems My Favourites The Today’s Assessments grid on the Assessments tab contains the following details: First column displays the priority, considering that you can submit only the first 12 row items for claims. Second column displays the diagnosis description, diagnosis code, and if entered, the status, side, and site details. Third column displays the differential diagnoses. Note: If an HCC column appears in the grid, the practice administrator must turn on the Hierarchical Condition Category (HCC) coding functionality in the Problems module. You can specify the columns, column order, and the type of description to display for search results in the diagnosis search window. The system retains the last display settings the next time the diagnosis search window launches. Select Preferences on the diagnosis search window. The Columns Configuration menu opens. To specify which columns to display, do one of the following: Select each column you want to display from the menu. A check mark appears to the left of the selected column to indicate that it is set to show in the grid. To hide the column and remove the check mark, you can select the column again. Select Risk Levels

You can indicate a patient's risk level using the Risk-Stratification template. Open NextGen® Enterprise EHR. On a patient encounter, open any of the standard visit templates (for example, the Intake) template. On the template header, select the Risk Level link. The Risk-Stratification template opens. Risk-Stratification Note: You can see the score in the Diagnosis based risk score field only when an HCC enabled diagnosis code is associated with the patient. Select the applicable column to assign a risk level to a patient. NextGen® Enterprise EHR automatically fills the Modified by and Date fields. Enter your comments.
