SOAP Link Icons
The icons for various links appear on the SOAP template. You can use various links options to perform the task quickly on the SOAP template. Icons Description The Provider Communication icon enables you to open the Provider Communication template to document the communication with a family member or another physician. This is an optional feature. The Meds icon enables you to open the Medication module to prescribe new medicines for the patient. The Procedures icon enables you to open a procedure list. This list is configurable. The All option can be added to the list to open all procedure choices or the navigation bar can be used to open the Procedures template. The Patient Plan icon enables you to generate the Patient Plan document, which can be printed and given to the patient at each visit. You can also generate Patient Plan from the Checkout tab. Generate Notes Associated with Encounters
You can generate notes associated with an encounter on the SOAP template. Open NextGen® Enterprise EHR. On the Intake or SOAP template, select Generate Notes in the Navigation Buttons section. Select the note to generate from the available list. Select OK. Documenting SOAP Information
Use the SOAP template to enter history, exam, assessment, and plan information. Reason for Visit on the SOAP Template You can use the Reason for Visit to enter the patient introduction and additional details about the reason for visit. Physical Exam Panel You can document physical exam details. Document Review of Systems Details You can review and update symptoms entered during the patient intake process in the Review of Systems section on the SOAP template. Review and Reconcile Allergies Review the Patient's Vital Signs Review Narratives on SOAP You can review and update the ambient-generated narrative text on SOAP. Document Social Confidential History
In the Histories template, scroll to the Social panel, and click the Confidential History link. The Confidential history template opens. Enter confidential history. Click Intake Note. Click the SOAP tab. To generate the Master document, do one of the following: Scroll to the bottom of the SOAP template, click Generate Note and select Master document from the list. Adding an Encounter Note
You can add an encounter note from a task for encounter, chart, and appointment source types. This option is available from the Notes tab on the Add/Edit Task window by right-clicking and selecting New > Encounter note. When you add encounter notes to encounter source types, the notes are added to the encounter. When adding encounter notes to a chart or appointment source type, the notes are added to the most recent encounter. Parent topic: Working Tasks Assigned to You

SOAP Template
Providers can do the following on the SOAP template. Review the patient details and add additional details. Document the presenting problems and various physical examination findings in the Physical Exam panel. Document the details of the performed procedures on the Procedures template. Document the assessments, follow-up details, treatment plans, lab orders, and office procedures in the Assessment / Plan panel. Review and update the narratives.
