In the
Histories
template, scroll to the
Social
panel, and click the
Confidential History
link.
The
Confidential history
template opens.
Enter confidential history.
Click
Intake Note
.
Click the
SOAP
tab.
To generate the Master document, do one of the following:
Scroll to the bottom of the
SOAP
template, click
Generate Note
and select
Master document
from the list.
Go to the left navigation bar and click
Preview
.
The
Master
document generates with confidential history listed.

Confidential Information
You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the following links. The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. The links allows you to document the following confidential information of the patient. Alcohol and drug use On the Confidential - Alcohol/Drug template. Psychiatric history On the Confidential - Psychiatric template. Abuse or domestic violence and incarceration On the Confidential - Abuse/Incarceration template. Sexual practices and STI details On the Confidential - Sexual Practices/STIs template. With the patient's permission, you can generate the documented confidential details in the social history document. You need to select the Include all confidential information in the social history document with patient permission checkbox on the templates to include

Social Confidential History
The Social Confidential history configured in the Practice Configuration and Document Configuration practice templates displays in the documents. The Confidential Social history can be configured to always display in the document in the Practice Configuration template. If the setting in the Practice Configuration template is not set to automatically display the confidential social history, then this information will not display in the document unless the checkbox is selected in the Confidential Social History template. Confidential Information
You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the Confidential History link in the Social panel on the Histories template. Patient's Social History
You can document a patient's social history. You can open the individual social history template or the Adult Social History template to document the social history information of a patient. The Adult Social History template includes the existing social history templates arranged in a one-page panel format. Each panel includes the data fields from the respective social history template. You can choose to document the social history information of the patient on individual social history templates or on the Adult Social History template. You can document the following social information for an adult patient on the social history templates or the Adult Social History template: Tobacco/Vaping Use Alcohol/Caffeine Statuses Lifestyle Occupation Comment Diet History Environmental Note: NextGen® Adaptive Content Engine is packaged with the individual social history templates as the default social history templates. Confidential Social History Configuration
The Show Confidential History checkbox in the Practice Configuration template enables you to include confidential information in the social history document with patient permission. Social History section in Practice Configuration When the Show Confidential History Information on Master Document checkbox under Practice Level Configurations in the Document Configuration practice template is selected, the confidential history information gets generated in the master or intake document. Practice Level Configurations in Document Configuration Note: When the Show Confidential History checkbox is selected on the Practice Configuration practice template, the system automatically selects the Show Confidential History Information on Master Document checkbox on the Document Configuration practice template. Generate the Intake Note Document
After documenting the patient information, the intake staff generates the Intake Note as the written summary of data collected in the encounter. The provider might also generate the Intake Note to obtain an overview of the collected information and then continue with the history of present illness, physical exam, assessment, and plan.
