Some of the data in the Treatment Plan panel is shared with the Individual Action Plan and IAP Review templates. Updates must be made from IAP or IAP Review templates by clicking the respective links. Any new goals, objectives and interventions must be entered here.
- In the Functional area field, do one of the following:
- Click to select a value from the list.
- Click the top, blank row from the list to type an entry in the field.
Making a selection from the Functional area list automatically opens lists for you to choose goals and objectives.
- Click the Goal field to select a pre-existing value based on the functional area selection.
This field is required before you can document data in the objective or intervention fields. If you attempt to enter the goal with an existing description, the following alert appears: A goal with this description already exists for this person. Please select that goal in the IAP or IAP Review to update status or other aspects of the goal as desired. This goal will not be added, as it is already present.
- In the Objective field, click to select a value based on the documented functional and goal values.
This required field is not available until Goal data is present. Another alert launches if you try to enter a previously recorded Goal with the same Objective: An objective with this description already exists for this person and this goal. Please select that objective in the IAP or IAP Review to update status or other aspects of the objective as desired. This objective will not be added, as it is already present.
- Type up to 200 characters describing the client's planned Intervention in the associated field.
This field is read-only until there is data present in the Goals and Objectives fields. Attempting to enter the recently recorded Goal and Objective with an already documented Intervention prompts the following alert: An intervention with this description already exists for this person, this goal, and this objective. Please select that intervention in the IAP or IAP Review to update status or other aspects of the intervention as desired. This intervention will not be added, as it is already present.
- To delete the field data before it is saved to the grid, click Clear.
- Click Add To Grid to enter the data into the read-only grid.
After adding a Goal or Objective to the grid, the filter automatically defaults to a status of New. Adding an Intervention automatically defaults the grid filter to Active.
- If appropriate, filter the grid using any of the following fields:
- Display goal status of
- Display objective status of
- Display intervention status of
- In the Orders/Assessment/Plan section, click Add to open the My Plan tabbed template set on the My Plan tab.
Note: There is an Practice Configuration template setting that determines if the My Plan or My Custom Plan template opens on the My Plan tab. Refer to the My Plan User Guide for NextGen® Adaptive Content Engine to find out more about this functionality.
- Click Add Plan Instructions to open the Ord BH Plan template.
Data entered on the Ord BH Plan template shares onto the Orders module and Assessment/Plan grids found on many of the Behavioral Health templates.
- In the Plan/additional information field, click to type or dictate a memo.
- Click the My Phrases link to select a sentence to enter in the Reason for referral field.
- In the Treatment is necessary to section, select one of the following options if they apply:
- Reduce risk of client needing a more intensive level of care (e.g. hospitalization, relapse)
- Reduce risk of harm to self or others
- Maintain or improve current level of functioning
