Enter Goals, Objectives and Interventions to be Addressed in the Current Shift
The first panel on the BH - Residential Shift Note template documents the goals/objectives and interventions addressed in the current shift. Required fields are marked with an asterisk.
Open the BH - Residential Shift Note template and expand the Goals/Objectives and Interventions Addressed this Shift panel.
Under the first section, click the Addressed diagnosis field to open an associated list.
The Diagnosis code field automatically populates and vice versa if the user decides to use the code field to enter the data.
The Addressed Diagnosis or the Diagnosis code fields are not required and launch an Addressed Diagnosis list that pulls from the data previously entered in the Assessment/Diagnosis grid on other templates in the Behavioral Health template set.
Click the required Goal today field to open a list of new, continued, or revised goals entered on either of the Individualized Action Plan templates.
In the Objective today, required field, click to select a value from a list of all of the objectives for the client.
Click Add to populate the shift information in the grid.
Note: Attempting to add a duplicate of a goal and objective combination into the grid prompts an alert indicating a duplicate goal and objective cannot be added.
Click Clear to remove data from the fields before it is committed to the grid.
This grid shares onto the Shift 2 and Shift 3 section tabs.
To modify a grid entry, highlight the row to populate the fields above with the data, make the changes and click Update.
Click Remove to delete a highlighted grid row.
In the Interventions Addressed Today section, click the Select From Previous IAP Interventions link to open a list.
In the Interventions/methods provided field, do one of the following:
Select an intervention to display.
Use the My Phrases functionality to complete the documentation.
Enter the Response to intervention/progress toward goals and objectives field.
Enter Goals, Objectives and Interventions to be Addressed in the Current Shift
