Enter Assessment and Diagnosis Details

Expand the Assessment/Diagnosis panel. If enabled, click the DSM IV link in the upper, right corner to launch the BH – DSM IV template. Note: The display of the link is set up on the Behavioral Health section of the Practice Configuration system template. In the Behavioral Health Diagnoses section, select any of the available links. In the Diagnostic description required field, click to launch the diagnosis search window. Selecting a description value automatically populates the ICD Code field or vice versa. Add Status, Severity, Differential DX, and Impression.The out-of-the-box Severity picklist does not include Extreme or Persistent. These values can be added to the list in File Maintenance. Enter any required Specifiers by doing one of the following actions: Click the field to type a value. Click the My Phrases link to create a new sentence or part of a sentence. Document Treatment Recommendations/Assessed Needs

On the Treatment Recommendations/Assessed Needs panel, if appropriate, select No additional recommendations clinically indicated. To create a new entry, click the Priority field to enter a number. Type an Assessed Need. In the Status field, click to select a value from the Need status list. Enter a date in the Identified field. If applicable, enter Rationale(s) and/or a Resolved date. Click Add. Repeat steps 1 - 6 as needed. In the Further Evaluations Needed section, do one of the following: Select None indicated. Place a check mark next to any of the applicable options. Enter details in the Level of care/indicated service recommendations field. Type a summary in the Individual served/guardian/family response to recommendations field. In the Treatment Planning Updates section, select No or Yes to Change in IAP Required. If Yes is selected, complete the IAP revision by clicking IAP Review at the top of the template. Document Assessed Needs, Goals, Objectives, and Interventions

Any previously entered data fills the grid under the Assessed needs section on the panel. On the Assessed Needs, Goals, Objectives and Interventions Overview panel, click the Display status of field and select a value to sort the grid. Click the Add button beneath the grid to enter an Assessed need on the template. In the Priority field, click to type a number. Type an explanation in the Assessed Need field. In the Status field, select a value from the Need status to display list. Enter a date in the Identified field. If applicable, set the Resolved date. If the need has been declined, deferred, or referred out, enter a Rationale. Click Save. Do one of the following: Click Clear for Add to enter other assessed needs. Click Close to return to the IAP tab. Enter Goal Information

When populating data in the Goal Information panel, it is important to note that need and intervention data is not mandatory to complete the plan, but, goals are required. Assessed needs can also be entered on the Comprehensive Assessment template, on the Assess/Plan tab, Treatment Recommendations/Assessed Needs panel, during the intake process. In the Goal Information panel, click the Linked to assessed need field to highlight a value in the resulting list and then click OK.The read-only From form dated and Need source fields pull from information entered during the Comprehensive Assessment exam. In the Functional area field, click to select a value from the list. In the Goal field, select an item that is associated with the assessed need or manually type a goal. Set the Start date and a Target completion date for the goal. If the start and target dates are not entered before selecting an Adjusted target date, the system throws an alert. Enter Treatment Plan Information

Some of the data in the Treatment Plan panel is shared with the Individual Action Plan and IAP Review templates. Updates must be made from IAP or IAP Review templates by clicking the respective links. Any new goals, objectives and interventions must be entered here. In the Functional area field, do one of the following: Click to select a value from the list. Click the top, blank row from the list to type an entry in the field. Making a selection from the Functional area list automatically opens lists for you to choose goals and objectives. Click the Goal field to select a pre-existing value based on the functional area selection. This field is required before you can document data in the objective or intervention fields. If you attempt to enter the goal with an existing description, the following alert appears: A goal with this description already exists for this person. Please select that goal in the IAP or IAP Review to update status or other aspects of the goal as desired. This goal
