Document Medication Information

On the BH - Psychopharm template, expand the Medication Information panel. Answer the following questions with Yes, No, or Not applicable as applies: Takes meds as prescribed Changes in medical status Other meds Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open the Medication Side Effects - BH template. Entering information on the template automatically selects the Yes option in the Medication Information panel and turns the Side effects reported link purple to indicate that data is present from that link. At the top of the grid, click the Comments link if additional information is required. All prescriptions currently existing in the Medications module are visible in the grid under the Active Medications section. If a medication is Inactive, a stop date appears in the grid. On the BH - Nursing Note template, navigate to the Medication Information panel and answer the following questions with No, Yes, or N/A (Not Applicable) and enter Comments as needed: Takes medications as prescribed Changes in medical status Reviewed medication name(s), dosage, purpose, and frequency Were medications delivered today - If yes, for what duration Allergic reactions Side effects reported Note: All allergic reactions must be entered in the Allergies module to ensure that they are apparent on future visits. Click the Side effects reported link to open a template on which medication side effects can be documented. Next to Other medications, select from the following options: None Herbal Over the counter Other - and type specifics in the associated field Click the Office Procedures link to launch the My Plan tabbed template set on the Office Procedures tab and document medications administered today. Enter Medication Information

In the Active section on the Medication Information/Consent panel, select the Comments link to include relevant details. To enter medications, select Add/Update. The Medications module opens. Any medications you enter display in the medications grid on the Psych Eval (BH) template. To reconcile medications, select Reconcile. The Medication Review window opens and you can complete the reconciliation process. In the Active Medications grid, highlight a medication. The medication details display in the Medication field in the Medications Consent section. In the Target Symptoms for Consent field, enter text. In the How it was discussed field, select a method from the list. Enter the Patient/Guardian Initials and the Provider Initials. Select Add. The details display in the grid. In the Other Medications Reported by Patient and Comments on past medications fields, document medications not prescribed by the prescriber or facility. Record Medication Data

On the BH - Psychiatric Evaluation template, expand the Medication Information panel. In the Active Medications section, select a Patient status. If required, click the Quality button for information associated with the Clinical Quality Measure (CQM). Click the Comments link to include medication details in the generated document. Select one of these options: No medications this encounter Medications reconciled Click Add/Update or Reconcile to update the data in the grid. To include Other Medications Reported by Patient, use the Add, Update, Remove, or Clear buttons. CAUTION: It is highly recommended that the Other Medications Reported by Patient values are verified for accuracy before entering them into the grid for Active Medications. Care Guidelines Grid View

You can follow a disease or condition and health maintenance during a patient encounter on the Care Guidelines grid view template. The Care Guidelines grid view template displays only active guideline items that are due in a data grid in the Care Guidelines panel. Workflow on the Care Guidelines grid view template starts in the Care Guidelines panel. To use the care guidelines system with a patient, you have to activate a guideline. Then you can perform the tasks of reviewing and updating the status of individual guideline items in the Care Guidelines panel. Viewing Patient Medications

You can view the patient's medications in the Medications panel. You can view the patient's active medications and the medications prescribed, modified, and renewed during the encounter in the Medications panel. You can prescribe additional medications, and reconcile them with the medications the patient is consuming. The No medications this encounter checkbox is encounter-based. If you select the No medications this encounter checkbox on a previous encounter and then select the Prescribed Elsewhere checkbox to prescribe medications in the current encounter, the No medications this encounter checkbox for the previous encounter remains selected. Patients can view their active and prescribed medications for the encounter in the documents.
