Select No or Yes for H&P completed within 30 days of scheduled procedure date.
If you select No, an alert message appears with the timeframe to complete the medical health and physical examination.
If you select Yes, the calendar opens.
Select the date when the medical health and physical examination was completed and select OK.
The selected date appears in the Date completed field.
Select the Mode of ambulation field and select the patient's appropriate condition for ambulation.
Enter any Comments on pre-procedure care.
You can select the My Phrases link select a common phrase to add in the Comments field. You can use the My Phrases link at the top-right of the Comments field to add comments.
