Record Vital Signs for the Nurse Progress Note
On the Vital Signs panel, click Historical information entered this encounter to view all vitals for the current encounter. Click History to view a complete history of the client's vitals. Click Add to add a new set of vitals. The Vital Signs (Lite) - [New Record] template appears. The Measured date, Time, and Measured by fields are populated. However, the date and time can be manually changed but the user name in the Measured by field cannot be modified. Enter information using the on-screen numeric keypad. Click the NEXT button below the numeric keypad to enter information in the next field. Select the Carried forward option to pull a previously entered Height into this field. After a Height and Weight value are entered, a BMI value is calculated below the numeric keypad. Select the BMI Plan link to open the Health Promotion Plan template, where an out-of-range BMI can be easily addressed with diet, exercise, and referral information. Enter Goals, Objectives and Interventions Addressed Today
Goals and/or objectives that were entered previously on the Comprehensive Assessment templates populate in this panel. The grid on this panel offers a quick view of the client's current goals and objectives. In the Goals/Objectives Addressed As Per Individualized Action Plan section, Goal today field, click to select a previously entered item. Select the Objective today from the list previously entered objectives. Click Add. In the Interventions Addressed Today section, click Select From Previous IAP Intervention to choose a previous entered value. Enter the Interventions/methods provided. Record the Response to Interventions/progress toward goals and objectives. Document the Issues referred to physician/psychiatrist for consideration. Document Plan and Additional Information
On the Plan and Additional Information panel, do one of the following actions: Click the Plan/additional information field to manually type relevant information. Use the My Phrases functionality to enter a sentence or part of a sentence in the field. Assessment and Plan Details
The A/P Details link in the left side navigation opens the Assessment/Plan Details and Treatment template. Use this template to add as many impressions, comments, differential diagnoses, and patient and provider’s plans for a diagnosis, as necessary. Enter Additional Assessment and Plan Details
You can document additional information such as a differential diagnosis, plan details, or comments for the assessment on the Assessment/Plan Details template. On the My Plan (PT) template, click the A/P Details tab.The Assessment Plan Details template opens. Assessment Plan Details To add or update the working assessment, select an assessment in the Today's Assessments grid. Type or dictate patient data in the Impression/Comments, Plan Details, Differential Diagnosis, and Provider Details. For a follow-up visit or disposition is needed, place the order.
