Enter Assessment and Diagnosis Details

Expand the Assessment/Diagnosis panel. If enabled, click the DSM IV link in the upper, right corner to launch the BH – DSM IV template. Note: The display of the link is set up on the Behavioral Health section of the Practice Configuration system template. In the Behavioral Health Diagnoses section, select any of the available links. In the Diagnostic description required field, click to launch the diagnosis search window. Selecting a description value automatically populates the ICD Code field or vice versa. Add Status, Severity, Differential DX, and Impression.The out-of-the-box Severity picklist does not include Extreme or Persistent. These values can be added to the list in File Maintenance. Enter any required Specifiers by doing one of the following actions: Click the field to type a value. Click the My Phrases link to create a new sentence or part of a sentence. Pain Management

You can document pain management information for the patient in the Health Promotion Plan template. Scroll down to find and expand the Pain Management panel. Pain Management panel Enter data in the Pain Assessment/Plan of Care fields. Type any comments or use My Phrases to add plan details in the Follow-up plan of care field. Note: My Phrases accessed from this grid are not NextGen® Orthopedic Suite phrases, they are My Phrases that have been set up in the NextGen® Adaptive Content Engine. Enter an Interim Update

On the Interim Update panel, do one of the following: If there were no modifications, click the No changes reported/observed checkbox. If changes exist, manually type your comments in the Interim update field or use the My Phrases functionality to complete the documentation: Click the My Phrases link to select a sentence to enter in the Reason for referral field. Enter Plan of Care and Additional Information

In the Plan and Additional Information panel, click My Plan to open the My Plan template set. My Plan Template Enter the details of an action plan that can be implemented by the client between sessions, in the Plan/additional information field. Enter the Prescriber's Evaluation

Expand the Prescriber's Evaluation panel. If there are no modifications, select the No changes reported/observed checkbox. In the Interim Update field, use the My Phrases link or manually type information about the client's progress since the last visit related to symptoms, effectiveness of medications, substance use, changes in family/social history, overall functioning, and any new issues. Patient's Treatment Plan

The treatment plan for the patient can be documented on the My Plan or My Custom Plan template. The My Plan template opens within NextGen® Adaptive Content Engine by default and can be replaced with the My Custom Plan template based on the configuration in the Practice Configuration practice template. The My Custom Plan template can be customized to suit your practice's needs and is less select-intensive.
