Reasons for Visit of Patients

You can document a patient's reasons for visit. On the Reason for Visit panel, you can document the common reasons for a patient visit. You can view the configurable specialty-specific reasons for visit list on the left pane. When you select a reason for visit that is already selected in the current encounter, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can select up to 14 History of Present Illness (HPIs) simultaneously. Additionally, you can select the Do not launch HPI checkbox and any HPI to open the corresponding HPI template. Note: You can configure sub menu items for the reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the sub menu items appear in the Reason for Visit panel on the left pane under the respective HPI. Add a Reason for Visit

You can add up to six HPIs as the reason for a patient visit for an encounter. Open the Intake template. Expand the Reason for Visit panel. To do a full skin exam, in the Complete skin exam desired? question, click the Yes option. To add a reason for visit without additional details select the Do not launch HPI check box. To add a reason for visit with additional details on the HPI template, clear the Do not launch HPI check box. In the left pane, click the needed entries.The selected reasons for visit appear in the Reason for Visit column. If the reason for visit is not listed in the panel, do the following: Click the Other list and select the reason for visit from the resulting list. You can also type a reason in the Other field and click Add. To document any details, click the Intake Comments link.The Intake Comments window opens. Reason for Visit on the SOAP Template

Open the SOAP template. Do one of the following: If the Introduction field contains text, review the content and make any necessary changes. If the Introduction field is blank, compose an introduction for the patient. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If checked, clear the Do not Launch HPI check box. To add additional details to a previously entered reason for visit, click one of the reasons for visit in the data grid.The appropriate HPI template opens. To add a new reason for visit: Select a complaint from the reasons for visit in the left pane. In the Other field select a reason from the resulting list or type a reason for visit.The associated HPI template opens. On the HPI template, enter the appropriate data. Enter any comments. Set Up HPIs to Appear in the Reason for Visit

You can set up the History of Present Illness (HPI) to appear in the reason for visit. A practice can configure the Reason for Visit panel using the Framework Content system template. For more information on the reason for visit configuration, go to NextGen Healthcare Success Community, and download the latest Framework Content System Template for NextGen® Adaptive Content Engine. Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. In the Show section, select All. To open the Framework Content template, double-click Framework Content. Select Specialty and select a specialty from the list. Expand the Reason for Visit panel. In Filter, select the all option. Select the reason for visit linked to the appropriate History of Present Illness (HPI), which you want to add to the all list in the Reason for Visit list. Document Patient Reported Glucose Results

On the Chronic Conditions - Hpi template, navigate to the Labs section. To document and graph the patient’s reported glucose results, select the Home Glucose Readings link.The Home Glucose Map template opens. To enter values, do one of the following actions: Select Download Meter Data. Select the Date field to enter the client reported value and continue to enter data points in the resulting windows. To create a visual representation of the data, select the Graph Daily Readings and Graph Min., Max. & Average. Select Save & Close to return to the Chronic Conditions - Hpi template.
