Chronic Conditions Data

The Chronic Conditions and Chronic Conditions HPI templates are used for documentation of multiple chronic conditions in one place, including diabetes. In addition, you can review health maintenance items, order tests, and review recommended care items and patient goals and/or self-management. Note: The Chronic Conditions and Chronic Conditions HPI templates cannot be used on the same encounter. To use the Chronic Conditions template, be sure that chronic conditions is not selected in the Reason for Visit panel. Diabetic Foot Exam

Providers can document a diabetic foot exam and a Monofilament exam on the PE Foot Diab Screen template. The Diabetic Foot Exam link is located in the Physical Exam panel on the SOAP template. There are sections for risk assessment, footwear evaluation, and counseling. Parent topic: Diabetes Care

Enter History of Present Illness (HPI) for Diabetes

Open the Intake template. In the Reason for Visit panel, select diabetes.The Diabetes Mellitus - HPI template opens. In the Managed With section, select the Fingerstick blood sugars checkbox. The DM Fingerstick Readings template opens. Enter the fingerstick readings and select OK. Select the field under the Fingerstick blood sugars checkbox and do one of the following in the Managed with list: Select one or more diabetic treatments in the list. To enter the treatment details manually, select the blank space in the list. Note: If you hover over the field, a tool tip displays the added treatment details. You can select the Cl button to remove the treatment details added in the field. The selected diabetic treatment details also appear in the master document. Enter additional HPI details as needed. Select Save & Close.Documented HPIs along with the diabetic treatment details appear in the Reason for Visit panel. Parent topic: Diabetes Care

Document Diagnostic Study Results

You can document the diagnostics study results in the Diagnostic History Entry template. Open NextGen® Enterprise EHR. Open the Histories template. In the Diagnostic Studies panel, select Add.The Diagnostic History Entry template opens. Select a diagnostic study from the grid. In the Study Result section, select Interpretation and select a status. In Result, enter a brief synopsis or interpretation of the result. In Comments, enter your comments or use the My Phrases link. Select Add to HPI Grid.The selected diagnostic study is added to the Reason for Visit panel as Studies Reviewed. Note: The Add to HPI Grid button is allowed only if the selected study is created in the current encounter and has a documented Result or Interpretation. Select OK. Enter Studies in the Diagnostics Tab You can enter the patient’s insurance information and policy number if they have been previously entered into the system using the Diagnostics Studies template.
