Access the Onc Treatment Summary Template
Open a patient encounter and select a Specialty of Oncology and a Visit Type of Office visit. Note: Ensure the patient has an appropriate cancer diagnosis code in the Billing ICD list. The diagnosis code must match a valid CRR Report Code. Document the condition in the patient’s Problem List module. Select Cancer Treatment Summary from the navigation bar. The Onc Treatment Summary template appears. Add Notes
Access the Onc Treatment Summary template and scroll to the Notes panel. Notes Click the Type field and select a note type from the list. Note: My Phrases functionality is keyed to the selected note type enabling you to define a separate set of My Phrases for each type of cancer note. For more information on managing and using the My Phrases functionality, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen Adaptive Content Engine. Type notations about the patient or the patient's condition or disease into the Note field. Note: Use the Clear button to clear the data entry fields of all data. Click Add. The system adds the note to the Notes grid. Note: The Notes grid only shows Notes added during the current encounter. To view notes from a previous encounter, you must open a previous encounter or generate a document. Specify Cancer Medications
Access the Onc Treatment Summary template and scroll to the Oncology Medication History panel. Oncology Medication History Click the Medication name field and select an active medication from the list. Note: The list shows all active and inactive medications for a patient. You can only select active medications from the list. If you select an inactive medication you will be prompted to re-prescribe the medication in the Medication module. You must first enter a medication in the Medication module for it to appear on this list. Enter data in all the remaining fields and click Add. The system adds the record to the grid and clears the data entry fields. Add Staging Detail
You can add additional staging details for these cancer types: Breast Cancer Lymphoma Prostate Cancer Soft Tissue Sarcoma Testicular Cancer Small Cell Lung Cancer Chronic Myelogenous Leukemia Access the Onc Treatment Summary template and scroll to the Staging panel. Click Add under the Staging grid. The Cancer Details template appears. Note: All three Add buttons in the Staging panel open the Cancer Details template. Expand the Staging Detail panel. Staging Detail Go to the section on the panel for the specific cancer type and complete the fields for which you have data. Optionally, click the Comments field and enter additional notations regarding the patient or patient's condition. Click Save & Close. You are returned to the Onc Treatment Summary template. Remove Notes
Access the Onc Treatment Summary template and scroll to the Notes panel. Select the note to be removed in the Notes grid. The data entry fields are populated with the data from the selected note. Click Remove. Parent topic: Add Notes See also Access the Onc Treatment Summary Template

Add Social History
Before you begin You can configure the show confidential history, panel version of adult and pediatric social history templates from the Social History section of the Practice Configuration template. For more information about these settings, go to NextGen Healthcare Success Community and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine You can add social history information in the History Summary section. The Social section uses age-specific templates based on the patient age and the Pediatric Age configuration on the Practice Configuration practice template. Open NextGen® Enterprise EHR. Open the Intake template. In the History Summary section, select Social. Select Select Social Hx to Add/Edit and select the relevant option from the list.Based on the patient age, the relevant social history template opens. Document the reported history information.
