Add New EM Coding Bullets

If the CMS Guidelines change or additional EM types are needed, NextGen Healthcare makes updates to the E&M Bullets and Field Mapping template in subsequent releases. If modifications or additions are to be made to the information found on this template, the following instructions describe how to make these changes. On the E&M Bullets and Field Mapping system template, access the EM CODING BULLETS section. To add a new EM coding bullet, do one of the following: In the Edit Fields grid, add a new row by manually entering the data or selecting the options from the drop-down arrows in the edit fields above the grid. Click the Add button. Note: The Add button is visible only when you add information in the fields. The new row appears in the data grid. Add a new row by editing a similar item already listed in the data grid and adding the edited information to the data grid: Select the applicable row in the grid. The Edit Fields found above the data grid populate with the corresponding data.

Delete an EM Coding Bullet

CAUTION: You must use caution when deleting rows from the EM CODING BULLETS data grid. On the E&M Bullets and Field Mapping system template, access the EM CODING BULLETS section. In the Edit Fields grid, select the applicable row. The Edit Fields found above the data grid populates with the selected data. Click the Delete button. Note: The Delete button is visible only after a row has been selected from the data grid. The row no longer appears in the data grid. Update an EM Coding Bullet

On the E&M Bullets and Field Mapping system template, access the EM CODING BULLETS section. In the Edit Fields grid, select the applicable row. The Edit Fields found above the data grid populate with the selected data. After making the necessary changes to the fields above the data grid, click the Update button. Note: The Update button is visible only after a row has been selected from the data grid and new data entered. The selected row displays the new information in the data grid. EM Coding Bullets

The EM CODING BULLETS or Page 1 (Bullets) section is pre-loaded with all the Evaluation and Management (E & M) Guidelines for all HPI elements, ROS elements, Histories elements, and Exam types found in the NextGen® Adaptive Content Engine. The system assigns each element a bullet ID based on the Centers for Medicare and Medicaid Services (CMS) Guidelines. EM Bullet ID - Field Mapping

The EM BULLET ID -FIELD MAPPING or Page 2 (Mapping) maps the EM Coding Bullet ID to a specific field on template. If any of the mapped fields are documented during an encounter, the EM Bullet ID is tallied with other bullets. A total tally of all documented bullets is shown on the EM History template. If new templates are added, or if existing templates are customized, and they contain HPI, or physical exam fields, the fields need to be mapped using Page 2 (Mapping) section of the E&M Bullets and Field Mapping template to be included in the EM calculation. For HPI’s, it is recommended that you use an existing HPI template as your starting point to minimize the need for EM mappings. For the history fields, it is recommended that you share the radio buttons and doc gen hidden fields from Master_IM onto your new template. By doing so, no additional mappings are needed. Show Single-system Physical Exam Field Mappings

You can view single-system physical exam field mappings on the Evaluation and Management (E&M) bullets and field mapping system template for 97 coding guidelines. Under the 97 Coding Guidelines, you can view physical exam field mappings on the E&M Bullets and Field Mapping system template for the following Single systems: Ears, Nose, and Throat (ENT or Otolaryngology); Dermatologic; Genitourinary; Musculoskeletal; Neurological; and Psychiatric.
