Physical Exam Elements for Evaluation and Management Coding

Under the 95 and 97 Coding Guidelines, each relevant exam field on the physical exam templates has been assigned an EM Bullet ID on the E&M Bullets and Field Mapping system template. This template stores all 95 and 97 E&M Coding Guidelines for Multi-and Single-system exams and their field mappings. A stored procedure and additional triggers on the Finalize template and on the Coding OPH template for NextGen® Adaptive Content Engine, 8.4.3 or earlier, total the field mappings for each system and calculate the suggested exam level component of E&M coding. EM Bullet ID - Field Mapping

The EM BULLET ID -FIELD MAPPING or Page 2 (Mapping) maps the EM Coding Bullet ID to a specific field on template. If any of the mapped fields are documented during an encounter, the EM Bullet ID is tallied with other bullets. A total tally of all documented bullets is shown on the EM History template. If new templates are added, or if existing templates are customized, and they contain HPI, or physical exam fields, the fields need to be mapped using Page 2 (Mapping) section of the E&M Bullets and Field Mapping template to be included in the EM calculation. For HPI’s, it is recommended that you use an existing HPI template as your starting point to minimize the need for EM mappings. For the history fields, it is recommended that you share the radio buttons and doc gen hidden fields from Master_IM onto your new template. By doing so, no additional mappings are needed. Evaluation and Management Codes

The E&M Bullets and Field Mapping system template enables you to create and manage Evaluation and Management (EM) codes within the NextGen® Adaptive Content Engine. This includes adding and modifying an EM type, bullet ID, EM field maps, and EM table list. The data configured on the E&M Bullets and Field Mapping system template is used to calculate the Evaluation and Management Code. The system is packaged with preconfigured values for the NextGen® Adaptive Content Engine content, where applicable. EM Coding Bullets

The EM CODING BULLETS or Page 1 (Bullets) section is pre-loaded with all the Evaluation and Management (E & M) Guidelines for all HPI elements, ROS elements, Histories elements, and Exam types found in the NextGen® Adaptive Content Engine. The system assigns each element a bullet ID based on the Centers for Medicare and Medicaid Services (CMS) Guidelines. Add a New Bullet ID - Field Mapping

On the NE&M Bullets and Field Mapping system template, access the EM BULLET ID - FIELD MAPPING section. If an EM Type is not currently selected, select an option from the drop-down arrow. Select the table that contains the exam fields to be mapped from the drop-down in the Table field. The data grid displays a list of fields found in the selected table. Note: Once a table is selected, only the fields that are native to that table are listed in the Mapping data grid. Non-clinical fields may also be listed in the Mapping data grid, so not all fields need to be mapped for any particular template. In this example, the EM Type has already been selected. If the desired table is not listed in the picklist, click the Page 4 (Tables) link to add the table to the EM Table List. Click the Table field to select an option from the picklist.
