Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. Dynamic HPI Templates

In the NextGen® Adaptive Content Engine templates, the following types of History of Present Illness (HPI) templates are used to document a patient's reason for visiting the office: Static HPI - displays hard-coded content that is not easily customizable; a single template is available for each complaint with options for selecting pre-determined details: the final concatenation is available when you close the template Dynamic HPI - enables you to add, edit, update, and delete dynamic content for the HPI and concatenates the content on the template as items are selected or edited. NextGen® Adaptive Content Engine is packaged with pre-configured dynamic HPI templates for the Gastroenterology, Physical Therapy, Orthopedics, Urology, and Pediatrics Well Child. Patient's Reason for Visit

You can document a patient's reasons for visit. In the Reason for Visit panel, you can document the History of Present Illness (HPI), problems, and diagnosis history as the reason for a patient visit. When you select a reason for visit that is already selected in the current encounter, an alert appears indicating that the reason for visit is already selected. You can either enter the duplicate reason for visit, or remove the duplicate. For a reason for visit that has separate listings for different specialties, such as abrasion, if you select two HPIs from different specialties for the same reason, an alert appears. You can select up to fourteen HPIs simultaneously. Additionally, you can select the Do not launch HPI checkbox and can select any HPI to open the corresponding HPI template. Note: You can configure sub menu items for reason for visit on the Framework Content system template for the specialty. Templates Used to Customize a Dynamic HPI

You can manage History of Present Illness (HPI) sections and findings in the dynamic format on five practice templates and one system template. Dynamic HPI Config Practice Template

The Dynamic HPI Config practice template enables you to configure elements for the dynamic History of Present Illness (HPI) templates. Each dynamic HPI template is linked to a specific reason for visit. The dynamic HPI templates that are packaged with NextGen® Adaptive Content Engine can be modified to meet the specific needs of a practice. If there is a need for a custom reason for visit, the practice can create a custom template. Data sets for labs, diagnostics, and procedures can be configured in the Section and Finding sections. New sections such as family history, lab data, and diagnostic data can be configured for a reason for visit. The new sections appear in the dynamic HPI template in the left pane under the Reason field in run time. The Contradiction setup link enables you to create guidelines that prevent users from selecting clinically contradictory findings.
