Document Vaccine Adverse Events
You can document the adverse effects of a vaccine.
- Open NextGen® Enterprise EHR.
 - Open the Immunization - Adult template.
 - In the Vaccine Adverse Event Report section, select Add. 
   The Vaccine Adverse Event Report window opens.
 - To enter the details of the adverse event, do the following:
   
- Select the onset date and time of the adverse event.
 - Enter the reaction to the administered substance.
 - Select the appropriate checboxes to indicate the result of the patient's illness.
 - Indicate where the vaccination is done.
 - Indicate how the vaccine is purchased.
 - Select the appropriate option to indicate the recovery of the patient.
 - If the patient had any illness at the time of vaccination, enter the details.
 - Enter the diagnostic test details, if any.
 - Indicate whether any adverse event is reported previously.
 - Enter the details of any family history of adverse events following vaccinations.
 
 - Select OK. 
   Note: If the patient has any medication or allergy related alerts, enter the details of the alert in the Alert field on the Immunization - Adult template.
