Document the Pediatric Patient's Historical Vital Signs
You can document the historical vital signs of a pediatric patient. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Document the patient's recorded historical vital signs. In Weight measurement, document the patient's weight in the lb and kg fields within the 4.409 – 1543.3 lb or 2.00 – 699.99 kg range. Note: A message appears if the value entered is not within the range. In Blood Pressure and pulse, document the patient's Systolic and Diastolic readings within the 0 – 399 range, and then document the patient's pulse rate within the 10 – 499 range. Note: A message appears if the value entered is not within the range. Click Measured date, and then select the corresponding historical date. To enter the time details, click Time, and then select the corresponding historical time. Click Save, and then click Close. The historical vital signs record appears in the

Document the Height of Pediatric Patients
You can document the height of a pediatric patient in the US or Metric designations. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. In the Vital Signs panel, select Add. The Pediatric [Age] Vital Signs template opens. In the Height/length measurements section, select Height, and then do one of the following: For a pediatric patient younger than two years, enter the height in the ft or in field within the 0 – 5.66 ft, 0 – 11.99 in, or 0 – 172.52 cm range, respectively. Enter a value in the total in field within the 0 – 67.92 in range. For a pediatric patient older than two years, enter the height in the ft, in or cm fields within the 0 – 11.99 ft, 10.16 – 406.4 in, or 0 – 9.99 cm range, respectively. Document Blood Pressure and Pulse
You can measure a patient's blood pressure and pulse. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. In the Vital Signs panel, select Add. The Pediatric [Age] Vital Signs template opens. Enter the blood pressure reading in the Systolic and Diastolic fields within the 0 – 399 range. Note: If the readings are out of the normal range, a message appears. Select a position and side. Enter the site where the blood pressure reading was taken. Select a method, and then select the appropriate cuff size. Document the Head, Waist, and Hip Circumference of Pediatric Patients
Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. In the Vital Signs panel, select Add. The Pediatric [Age] Vital Signs template opens. In the Head/Waist/Hip circumference section, do one of the following: For a pediatric patient younger than two years, enter the head circumference in the in or cm field within the 10.00 – 29.99 in or 25.4 – 76.17 cm range, respectively. For a pediatric patient older than two years, enter the head circumference in the in or cm field within the 10.00 – 29.99 in or 25.4 – 76.17 cm range, respectively. Enter the waist circumference in the in or cm field within the 10.16 – 406.4 cm or 4.00 – 59.99 in range, respectively. Enter the hip circumference in the in or cm field within the 4.00 – 159.99 in or 10.16 – 406.4 cm range, respectively. Document a Pediatric Pain Level
You can document a pediatric patient's pain level in the Pediatric Vital Signs template. Open NextGen® Enterprise EHR. Open the Intake template. In the Vital Signs panel, select Add. The Pediatric [Age] Vital Signs template opens. Enter the pain level. The default method Numeric Pain Intensity Scale appears in the Method field. You can select another method, if required. The selected pain level and method appears in the Screening Summary and Health Promotion Plan templates. To document the pain assessment and plan of care, click the Pain Plan link. The Health Promotion Plan template opens. On the Pain Management panel, the Pain level and Method fields automatically fills in. Select See HPI, if required. Document the Weight of Pediatric Patients
You can document the weight of a pediatric patient. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. In the Vital Signs panel, select Add. The Pediatric [Age] Vital Signs template opens. For a pediatric patient younger than two years, enter the weight in the lb, oz, or kg field within the 0 – 110.20 lb or 0.10 – 49.99 kg range, respectively. For a pediatric patient older than two years, enter the weight in the lb or kg field within the 2.2046 – 1543.2 lb or 1 – 699.99 kg range, respectively. Based on the value entered Document the Weight of Pediatric Patients in the lb or kg field, the NextGen® Enterprise EHR fills the other field automatically. A message appears if the value entered is not within the range. Select the Context option. You can open the Tare Weight link to record tare weight for durable medical equipment such as wheelchair, walkers, and so on,
