Document the Patient's Respiration and Pulse Oximetry

You can measure the respiratory rate and oxygen saturation level of a patient using the Vital Signs template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel and select Add. The age-specific vital signs template opens. In Respiration and Pulse Ox, enter the respiratory rate value in the Respiration field within the 0 – 99 range. Select the method of measuring the respiratory rate. Enter a value in the FIO2 field within the 20 – 100 range. Enter oxygen concentration for the patient's respiration. Enter the pulse oxygen rate during rest and ambulation of the patient within the 0 – 100 range. Note: A message appears if the value entered is not within the range. Enter the pulse oximetry resting and ambulation readings in the Pulse Ox Rest and Pulse Ox Amb fields within the 0 – 100 range. Select a source of oxygen supply. Select Pre-t

Document the Patient's Height

You can document the height of a patient in inches or in feet using the Vital Signs template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel. Select Add.The age-specific vital signs template opens. In Height/length measurements, do one of the following: To enter the current day's data, fill the ft, in, or cm fields within the 1 – 9.99 ft, 12 – 119.88 in, or 30.48 – 304.49 cm range, respectively. To enter previously documented data, select the Carried forward option. If you enter the height in either the ft, cm, or in field, the NextGen® Enterprise EHR fills values in the other fields automatically. Note: A message appears if the value entered is not within the range. Indicate the position of the patient when the height is measured. To enter the date when the patient's height is measured, select a date in Last Measured, or select

Document the Patient's Temperature

You can measure the body temperature of a patient. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel and select Add. The age-specific vital signs template opens. In Temperature, enter the patient's temperature in Fahrenheit or Celsius within the 50 – 121.98 F or 10 – 49.99 C range respectively. Based on the value entered in the F or C field, the other field is filled automatically. Note: A message appears if the value entered is not within the range. To document the physical site used to measure the temperature, select Site, and then select the appropriate method of measuring the temperature. Save and close the template. Document the Patient's Blood Pressure and Pulse

You can measure the blood pressure and pulse of a patient. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel and select Add. The age-specific vital signs template opens. In Blood Pressure and pulse, enter the blood pressure reading in the Systolic and Diastolic fields within the 0 – 399 range. Note: If the value entered is not within the range,a message appears to indicate that the value entered is out of range. The HTN Plan link opens the Health Promotion Plan template, where you can document a plan of care if the blood pressure of the patient is not in the normal range. Enter the pulse rate of the patient in Pulse within the 10 – 499 range. Note: A message appears if the value entered is not within the range. Open the Orthostatic Vital Signs Template

You can record a patient's orthostatic vital signs on the Orthostatic Vital Signs template. Open NextGen® Enterprise EHR. To open the Orthostatic Vital Signs template, do one of the following: On the Intake template, in the Vital Signs panel, select Orthostatic Vital Signs. On the Intake template, in the Vital Signs panel, select Add Vital Signs, and then select the Orthostatic Vital Signs link. Orthostatic Vital Sign Document Orthostatic Vital Signs You can measure and document the blood pressure and pulse rate of a patient in the lying, sitting, and standing positions for five, three, and one minute with any other associated symptoms and comments on the Orthostatic Vital Signs template. Remove Orthostatic Vital Signs Records You can remove the documented orthostatic vital signs records. Document Historical Vital Signs

You can document the historical vital signs for a patient. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel and select Add. The age-specific vital signs template opens. Document the patient's recorded historical vital signs. In Weight measurement, document the patient's weight in the lb and kg fields within the 4.409 – 1543.3 lb or 2.00 – 699.99 kg range. Note: An alert message appears if the value entered is not within the range. In Blood Pressure and pulse, document the patient's Systolic and Diastolic readings within the 0 – 399 range, and then document the patient's pulse rate within the 10 – 499 range.
