Document the Pregnancy Outcomes Template

You can document information in the Pregnancy Outcomes template. The Pregnancy Outcomes template provides you the ability to view and modify a patient’s gravidity and parity. You can view and modify the total parity values with or without the details of each pregnancy. Minimal required fields are Pregnancy Entry Type, Pregnancy #, and Delivery Type. It is preferable, on an OB initial visit to use the gravidity/parity fields and Pregnancy Outcomes templates from within the OB templates. When you activate parity auto-calculation, parity automatically calculates the details when you document historical pregnancy or end a current pregnancy When you select End current pregnancy the Gravidity reflects a non-pregnant status. This option is used on a postpartum or post-abortion visit and not on an initial prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select the Add/Update Detail button in the OBGYN Synopsis template.

Documenting Pregnancy Outcomes

You can document pregnancy outcomes in the OBGYN Synopsis and Pregnancy Outcomes templates. You can document the Pregnancy outcomes in the OBGYN Synopsis and Pregnancy Outcomes templates. Note: In a Gynecology patient, the OBGYN Details link would not be typically used in a pregnant patient. There are two ways to enter new pregnancy: Quick entry Detailed entry Quick Entry of Pregnancy Outcomes You can enter a quick parity entry in the Pregnancy Outcomes template. Detailed Entry of Pregnancy Outcomes You can enter a detailed parity entry in the Pregnancy Outcomes template. Document the Obstetrics and Gynecology Synopsis Template

The Obstetrics and Gynecology (OBGYN) Synopsis template provides the details of female patients. You can document information on the patient's menopause status, hysterectomy details, gynecological and pregnancy history, abnormal pap history, and contraception information for female patients over the age of eight years. You can use the OBGYN Synopsis template during a gynecology visit, not during an OB prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select from Primary OBGYN provider. Select from the following options. Detailed document: Select this to include the gynecological and pregnancy histories in the document. Reviewed, no changes: Select this option if you have reviewed the details documented in the previous encounter and no changes were made. Document a Pap Screening

You can view the Pap screening in the Pap Screenings panel. On the OBGYN Synopsis template, in the Pap Result, HPV Detail and Treatment grid, select Add/Update Detail.The Pap and HPV Details template opens. Pap and HPV Details Expand the Pap Screenings panel.If a Pap smear is present in the Orders module, the Pap smear date displays in the Done column and See module text displays in the Pap Results column of the Pap Result, HPV Detail and Treatment grid. Highlight the entry in the grid and edit any necessary information, based on the information provided in the Lab Order Results panel, and then select Update. Pap Screenings Select OK. Modify a Pap and HPV Screening, and a Treatment Record

You can modify a Pap and HPV sceening in the Pap Result, HPV Detail and Treatment grid. You can modify the information about a pap screening and treatment, as needed. On the OBGYN Synopsis template, in the Pap Result, HPV Detail and Treatment grid, select Add/Update Detail.The Pap and HPV Details template opens. Expand the Pap Screenings panel. In the Pap Result, HPV Detail and Treatment grid, do one of the following: To modify a pap screening, highlight the Pap and HPV screening record you want to modify, update the details in the Pap and HPV Screening section, and then select Update. Pap Result, HPV Details and Treatment To modify the treatment details, highlight the treatment record you want to modify, update the details in the Treatment section, and then select Update. Document the Treatment Section

You can document the treatment in the Treatment section. On the OBGYN Synopsis template, in the Pap Result, HPV Detail and Treatment grid, select Add/Update Detail.The Pap and HPV Details template opens. Pap and HPV Details In the Pap Screenings panel, in the Treatment section, select a Treatment date and then select Approximate performed date if the exact date is unknown. Treatment Section In Treatment Performed, select the applicable treatments performed. In Treatment results, select the results of the treatments.
