Document the IUD Removal Details

You can document the Intrauterine Device (IUD) removal procedure details in the IUD Removal section on the IUD - Procedure template.

1. Open the IUD - Procedure template.
2. In the IUD Removal section, select a diagnosis in Indication for procedure.
   

   If you select the Indication for procedure checkbox, the procedure and its code are automatically added in the respective fields.

3. Select a status for the diagnosis.
4. Enter the reason for removal, side effects, and the desired birth control method.
5. In Strings visible, select No or Yes.
6. Enter the instrument with which the Intrauterine Device (IUD) is removed.
7. Optional: Select the IUD removed without complications and Patient desires reinsertion checkboxes.
8. To document the physical exam details, select the Physical Exam link and document details in the GYN - PE template.
