Document the Implant Removal Details

You can document the implant removal procedure in the Implant Removal section on the Contraceptive Implant Procedure template.

1. Open the Contraceptive Implant Procedure template.
2. In the Implant Removal section, select a diagnosis in Indication for procedure.
   

3. Select a status for the diagnosis.
4. Enter the reason for removal, side effects, and the desired birth control method.
5. Select the implant type.
6. In Omplant placement was verified by palpation, select No or Yes.
   If you select Yes, you must select the applicable arm option.
7. Enter the name of the solution used in the skin prep.
8. Enter the anesthetic and dosage details.
9. In Implant successfully removed, select No or Yes..
10. Optional: Select the Patient desires reinsertion checkbox.
11. In the Applied section, select the Adhesive strip (advised remove in 3-5 days) and Pressure dressing (advised remove in 24 hours) checkboxes if needed.
