Document the IUD Insertion Details

You can document the Intrauterine Device (IUD) insertion procedure details in the IUD Insertion section on the IUD - Procedure template.

1. Open the IUD - Procedure template.
2. In the IUD Insertion section, select a diagnosis in Indication for procedure.
   

   If you select the Indication for procedure checkbox, the procedure and its code are automatically added in the respective fields.

3. Select a status for the diagnosis.
4. Optional: Select the History reviewed and no contraindications checkbox.
5. Enter the uterine size and uterus details.
6. Enter the name of the chemical used in cleaning and preparing the cervix.
7. Enter the Tenaculum site anesthetic and Paracervical block dosage details.
8. Optional: Select the Cervical dilation required checkbox.
9. Enter the uterus sound measurement in Uterus sounded to.
10. Optional: Select the IUD inserted in normal fashion with no complications and IUD successfully inserted per manufacturer’s instructions checkboxes.
11. Enter the strings trimmed measurement in Strings trimmed to.
