Document Responses for the Disability Screening Questionnaire

You can record a patient's responses to the American Community Survey (ACS) Disability Questionnaire in the Screening Tools template.

1. Open NextGen® Enterprise EHR.
2. Select a patient encounter, and open a template.
3. In the navigation bar, select Screening Tools.
   The Screening Tools template opens.
4. Select ACS Disability Questionnaire from the grid.
   The ACS Disability Questionnaire window opens.
5. Select the options for disability questions.
6. Select Save & Close.
   The Screening Tools Performed grid displays the ACS Disability Questionnaire.
7. Optional: To modify a record, select it from the Screening Tools Performed grid, select Edit, modify the details, and then select Save & Close.
8. Optional: To delete the record, select it from the Screening Tools Performed grid, select Remove, and then select Yes.
