You can measure and document the blood pressure and pulse rate of a patient in the lying, sitting, and standing positions for five, three, and one minute with any other associated symptoms and comments on the Orthostatic Vital Signs template.
- Open NextGen® Enterprise EHR.
- Open a patient's record, and then open an encounter.
- On Top Toolbar, select Templates
.
The Select Template window opens.
- Open the Intake template.
- Open a patient's record, and then open an encounter.
- On Top Toolbar, select Templates
.
The Select Template window opens.
- Expand the Vital Signs panel, and then do one of the following:
- Select Orthostatic Vital Signs.
- Select Add Vital Signs, and then select the Orthostatic Vital Signs link.
The
Orthostatic Vital Signs template opens.
Note: The date and time of the blood pressure and pulse measurement are displayed automatically.
- To remove previous data from the fields, select Clear for Add.
Note: If the orthostatic vital signs records are available for the current encounter, the Orthostatic Vital Signs template is filled with the corresponding values from the record.
- Select an option for the following:
- Side
- Method
- Position
Note: The default values for Position are Lying, Sitting, and Standing in the three columns, respectively. You can change the default values, which are then carried forward to future encounters.
- Enter the waiting time for blood pressure reading in the minutes field.
Note: The default time duration is five, one, and three minutes in the three columns, respectively. You can change the default duration. The updated values are carried forward to future encounters.
- Select Site, and then select a value from the list.
- Select an option for cuff size.
Note: The Large and Thigh options become available when you select Adult.
- In Blood Pressure and pulse, enter the blood pressure reading in the Systolic and Diastolic fields.
Note: If the Systolic and Diastolic value range is 0 – 399. An alert message appears if the value entered is out of range.
- In Symptoms, select and select a value from the list.
- Enter the pulse rate, and then select the pulse pattern within the 10 – 499 range.
Note: A message appears if the value entered is not within the range.
- Enter any additional information in Comments.
- Select Add.
The orthostatic vital signs recorded for the positions and durations appear in the grid.
- Save and close the template.
