Document Reasons for Visit

You can document the reasons for the visit of a patient in the Reason for Visit panel. Open NextGen® Enterprise EHR. Open the Intake – Cardiology template. Select the Reason for Visit panel. To add a Reason for Visit, do one of the following. To enter the reason for the visit and do not want to enter the history of present illness, select the Do not launch HPI checkbox. From the left pane, select a reason for visit. Click the Other list, select a reason for a visit from the list, and then click OK to get the corresponding window to fill in the information. The reason for the visit is added. To enter additional information for the provider to review, click the Intake Comments link, and type your comments.When you enter additional information, the Intake Comments link appears purple on both the Intake and SOAP tabs. Configure Copy Historical Free Form HPI Settings

You must enable the Copy Forward button on the Free Form HPI template to copy historical free form HPI narratives. Open NextGen® Enterprise EHR. From the File menu, click System \ Practice templates. On the System tab, double-click Configuration Home. On the left pane, click the Practice Configuration template. In the Cardiology section, select Enable Copy Forward FreeForm - HPI. Figure 1. . Reasons for Visit

The Reason for Visit appears on the left pane of the Intake-Cardiology template. You can customize the list by specialty or by provider. Reason for Visit panel on Intake - Cardiology template You can configure the Reason for Visit panel on the Intake – Cardiology template without logging on to Template Editor. You can configure or change the reasons for visit and the associated HPI templates using the Framework Content system template. Copy Reasons for Visit from Previous Encounters

You can copy reasons for a patient's visit from previous encounters to the current encounter instead of manually adding them. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. Select HPI History. The Historical Information - HPI window opens. Note: The records are displayed for the default duration configured in the Copy Previous Visit Information section of the Practice Configuration template. To filter the records in the grid, enter the search criteria in Filter by, and then select Search. Document Reasons for a Patient's Visit

You can document up to six reasons for a patient's visit during intake. Open NextGen® Enterprise EHR. Open the Intake template, and then expand the Reason for Visit panel. To add reasons for the patient's visit without opening another template, select Do not launch HPI, and then do one of the following: Select the reason for visit from the left navigation bar. To add a customized reason, enter the reason in Other, and then select Add. Select Additional / Manage, select appropriate reasons from the Reason for Visit window, and then select Save and Close. Note: You can view all the available history of present illnesses (HPIs) by selecting the All hpis checkbox in the Reason for Visit window. To add reasons for the patient's visit with additional details by using another template, clear the Do not launch HPI checkbox, select the necessary reasons, and then select Save & Close . The Reason for Visit panel is where the reason for visit, as described by the patient, is documented, along with an initial History of Present Illness (HPI). The provider should review reasons for visit, update if necessary, and complete HPI information. Reason for Visit Open NextGen® Enterprise EHR. If the Introduction field contains text, review the content and make any necessary changes.The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If the introduction field is blank, compose an introduction for the patient.
