Configuring Medical, Surgical and Family History

This document provides information on how to configure the specialty-specific medical history, surgical history, and family health history content. The Configure Medical, Surgical, Family Hx system template enables you to configure a list of medical, surgical and family health conditions, and management items that are associated with the SNOMED (Systematized Nomenclature of Medicine) codes. In run time, the configured conditions and management items appear in the Past Medical History and Family Health History templates. In the Configure Medical, Surgical, Family Hx system template, you can configure different types of conditions as described in the table. Condition and Management of Codes

You can configure the following in the Condition and Management Configuration panel: Conditions and management items with SNOMED codes for past medical history, past surgical history, management items, and family history.: HL7 codes for the social history and family history problem type Conditions and management items without SNOMED codes. Access the Configure, Medical, Surgical, and Family Hx System Template

To configure a system level medical, surgical, and family health history content, you must access the Configure Medical, Surgical, and Family Hx system template. In NextGen® Enterprise EHR, click the File menu and select System/Practice Template. In Select Template, click All to view a list of system templates. Select Configure Medical, Surgical, and Family Hx from the list and click OK. The Configure Medical, Surgical, and Family Hx template opens. Individual and Grouped Conditions and Procedures

Conditions of interest to a specialty appear on the Medical panel of the Past Medical History template and on the Family Health History template. Both templates enable you to document the patient's medical conditions and their family members medical history. Individual Conditions- Conditions listed without a plus sign [+] to the left of the item represent a single general disease or disorder. Grouped Conditions- Conditions listed with a plus sign [+] are types or groups that represent a category of diseases or disorders, with more than one associated disease or disorder as items to select from. On the Surgical panel of the Past Medical History template, surgery types or groups are similarly organized and identified with a [+], and items listed without a [+] are single surgical procedures. Note: Conditions or surgical procedures with an asterisk (*), indicate that the condition or procedure does not have a SNOMED code.
