Enter Crisis Treatment Plan
On the BH - Crisis Treatment Plan template, expand the Crisis Treatment Plan panel. Click the Suicide/Homicide Risk link to assess risk level. Crisis Treatment Plan The Safety Risk template opens. Safety Risk Click the Quality button to learn about the Quality Reporting requirements related to this type of risk. Click Save & Close to return to the BH - Crisis Treatment Plan template. Under Problem/Precipitating Events, manually type an explanation in the field. In the Signs of Risk field, enter a description of the signs, symptoms, and circumstances that may cause the documented risk to happen again. Under the Strategies to Reduce Risk section, use the Patient strategies and Staff interventions fields to document specific actions meant to lower the risk. In the Goals Prior to Release from Precautions section, record the target initiatives to be completed by the client before being released from precautions. Access the Crisis Treatment Plan

Document Professional and Agency Information for Patient Crisis Support
In NextGen® Enterprise EHR, in Patient History, select the Templates button. The Select Template window opens. On the Medical Records tab, select All. From the list, select Patient Safety Plan (BH) and select OK. Expand the Professionals/Agencies panel. Select the Community Resources link. In the Community Resources window, select the resource in the All Resources grid, and then select Include selected resource. The selected resource appears in the Patient's Resources grid. Select Save & Close. The resource information appears in the Professionals/Agencies grid. Emergency Contact/Patient Relationship/Support Roles
The Emergency Contact/Patient Relationship/Support Role panel enables you to add emergency contact and support role information that would play a role in the patient’s treatment. The data you enter into the Emergency Contacts/Patient Relationship data grids display in the Patient Information screens. The Support Resources data displays in the data grids that display in the Individuals Present panel on clinical templates. Document Individuals Present
In the Individuals Present panel, select from one of the Contact Type options. Select one of the following in the Individuals present section: Individual not present Individual present Select Others present and enter the details in the Support Resources section. In the Support Resources section, record the client's closest friends and relatives. To add the support resources data in the grid, click Add. To change the grid view, select All or Active. The Effective and End dates determine if a resource is Active.
