Add Patient and Provider Relationship

The Patient/Provider Relationship panel enables you to enter the names of individuals, who is working with the patient during treatment, and the role of each in the treatment of the client. Select Add. The Tis Edit Patient Prov Role window opens. Enter the Provider contact information, Program, Role, Effective and Expire Dates, and any additional Comments. Select Save & Close. Add a New Emergency Contact or Patient Relationship

Select Add. The Modify Patient Information window opens. Select the Relations/Role tab. In the Relations/Role section, select the Open Menu . Select New. The Relationship Lookup window opens. Do one of the following: To link to someone already in the system, search for the person and then select Open. To enter a new person, enter the person’s information and then select New. Managing Patient Demographic Information

In addition, you can use the Demographics (BH) template to enroll patients in programs, document COT/treatment adherence, support roles and resources, and the patient's employment details. You can select the Special Assistance checkbox if the patient needs any special care. Selecting the checkbox opens a picklist that you can configure in the BH Foundation Configuration template. After documenting the patient’s demographic information, you can select Generate Document to generate a Demographics document. Alternatively, you can select Save & Close. Document Emergency Contacts and Patient Relationship Details

You can document emergency contact details that would play a role in the participant's foster care program. The data you enter into the Emergency Contacts/Patient Relationship section is also displayed in the Patient Information window. Open the CW Demographics (BH) panel. Expand the External Relationship Information panel. In the Emergency Contacts / Patient Relationship section, select Add.The Modify Patient Information window opens. Select Relations/Role. Support Role Information

In the Support Roles section of the Relations/Roles tab, you can enter people that play an important role in the support of a person or patient in the practice. A support role can be a caregiver, an emergency contact, or a next of kin. Support roles and relationships are stored as separate demographic records in the database. Relationships are assigned a person number, and they are part of the Master Person Index (MPI). Relationships can be found from the People Lookup and Patient Lookup windows. Support roles are not assigned a person number, and they are not part of the MPI. Support roles cannot be found from the lookup windows. If a person is both a relationship and a support role for a patient in the practice, updating the demographic record for one does not update the demographic record for the other.
