Enter Start Time for Crisis Treatment Plan
The times entered on the first panel also appear in the Behavioral health billing section at the bottom of the template. The values can be edited in either place. It is important to follow the established format: 0:00 AM or PM. Expand the General panel. To populate the Start time field, do one of the following: Click the clock icon to enter the current time. Enter Crisis Treatment Plan
On the BH - Crisis Treatment Plan template, expand the Crisis Treatment Plan panel. Click the Suicide/Homicide Risk link to assess risk level. Crisis Treatment Plan The Safety Risk template opens. Safety Risk Click the Quality button to learn about the Quality Reporting requirements related to this type of risk. Click Save & Close to return to the BH - Crisis Treatment Plan template. Under Problem/Precipitating Events, manually type an explanation in the field. In the Signs of Risk field, enter a description of the signs, symptoms, and circumstances that may cause the documented risk to happen again. Under the Strategies to Reduce Risk section, use the Patient strategies and Staff interventions fields to document specific actions meant to lower the risk. In the Goals Prior to Release from Precautions section, record the target initiatives to be completed by the client before being released from precautions. Add Patient Relationship/Support Resources
On the BH - Crisis Treatment Plan template, expand the Patient Relationship/Support Role panel. To document the client's instructions for end-of-life care, click Advance Directives. This may include the name and contact information for a healthcare proxy and/or durable power of attorney. Click Add below the grid to document a new entry. To update an existing support, double-click that grid row and make changes as needed. The Patient Relationship/Support Role Info template launches. Enter Assessment and Diagnosis Details
Expand the Assessment/Diagnosis panel. If enabled, click the DSM IV link in the upper, right corner to launch the BH – DSM IV template. Note: The display of the link is set up on the Behavioral Health section of the Practice Configuration system template. In the Behavioral Health Diagnoses section, select any of the available links. In the Diagnostic description required field, click to launch the diagnosis search window. Selecting a description value automatically populates the ICD Code field or vice versa. Add Status, Severity, Differential DX, and Impression.The out-of-the-box Severity picklist does not include Extreme or Persistent. These values can be added to the list in File Maintenance. Enter any required Specifiers by doing one of the following actions: Click the field to type a value. Click the My Phrases link to create a new sentence or part of a sentence. Transfer or Discharge Visit Workflow
Set the Visit Type to Discharge Planning on the Home Page - Behavioral Health template to access the Transfer/Discharge tab, which includes three section tabs. The BH - Transfer/Discharge template set can also be accessed at any time by clicking Discharge/Transfer on the bottom section of the left navigation bar.
