Enter Historical Diet Details

You can document the diet details of the patient. The Social History – Diet template enables you to document specific diet details for breakfast, lunch, dinner, and snack consumed by a patient. The details on the patient's daily diet helps the clinician or provider to verify if a child gets adequate nutrition for normal growth and development. Open NextGen® Enterprise EHR. To open the Social History – Diet template, do one of the following: On the Histories template, expand the Social panel, select Diet History, and then click Add/Edit. In the History Summary panel on the Intake template, click Social, click Diet History, and then click Add/Edit. Social History – Diet Enter the date of the diet. In the Breakfast , Lunch, and Dinner section, do the following: Select the appropriate diet. Select the serving size and measure of the diet. Add Education and Leisure Activities Details

You can document the details of education and leisure activities of a patient on the Pediatric Social History – Education template. Open NextGen® Enterprise EHR. To open the Pediatric Social History – Education template, do one of the following: On the Histories template, expand the Social panel, select Education, and then click Add/Edit. In the History Summary panel of the Intake template, click Social, click Education, and then click Add/Edit. Pediatric Social History – Education In the Historian field, enter the relationship of the person who provides the patient information. In the Education section, do the following: Enter the patient's current school name and grade. Indicate if the patient has repeated any grades in school. If you select Yes, then enter the grade and the reason for the repetition. Select the grades the patient received in school. Document the Patient's Status

You can record the patient's status using the Social History - Status template. Open NextGen® Enterprise EHR. Open the Histories template. In the Social panel, select Statuses, and then select Add/Edit.The Adult Social History template opens. Expand the Statuses panel. In the Demographics section, enter the patient details. Select the Employment Information link, and on the Employment/Occupation template, select Add to create a new record. Select Save, and then select Close. Select the occupational hazards or enter the hazards manually.You can enter up to two hazards. In the Radiation Exposure section, enter the exposure date, radiation type, and the radiation dose and unit.By default, the encounter date appears as the exposure date. Document Vaping Details of a Patient

The Tobacco/Vaping Use panel in the Social History - Tobacco template enables you to screen and document the vaping details of a patient. The documented vaping details are carried forward to future encounters. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand the Vaping Use panel. In the Screened for vaping field, select Yes. Select a Status for vaping. If the patient is a current user, select a number in the Age started field. Document Discharge Details of Babies

You can provide the feeding history and the discharge details of a baby in the Discharge section on the Pregnancy or Birth History template. Open NextGen® Enterprise EHR. On the Intake template, click the Birth History link.The Pregnancy/Birth History template opens. In the Discharge section, select the Feeding history option. Click Formula type and select the infant feeding formulas from the list. Click Discharge date and select the discharge date of the baby. Figure 1. You can provide information on the tests done and the medications given to the baby in the Hospital course section on the Pregnancy/Birth History template. Open NextGen® Enterprise EHR. On the Intake template, click the Birth History link.The Pregnancy/Birth History template opens. In the Hospital course section, select No or Yes for the following fields: Hospital course Vitamin K injection Hep B vaccine Select Pass or Fail for Hearing test. If you select Fail, select Rt ear, Lt ear, or Both ears, and then select the Repeat hearing test ordered checkbox. Enter the infant blood type and select the Rh factor.
