Child Welfare Home Interview

You can do an in-depth interview of prospective foster parents. After the phone interview of prospective foster parents, you can do the home interview of the parents. The home interview is an in-depth interview of the prospective foster parents. During the home interview, the program staff determines the preferences of the prospective foster parents for foster placement and documents the following details about the fostering family on the CW Home Interview (BH) template. Child Welfare Summary and Case Closure

You can document the details when a parent (main participant) leaves the child welfare program. When a parent (main participant) leaves a foster care program, the providers document the discharge summary on the CW Summary/Case Closure (BH) template. The foster parent can leave the program for a variety of reasons. Parent adopted their foster child. Parent are no longer open to fostering or adoption. Parent are not eligible for foster care programs. CW Summary/Case closure Panel You can use the Signature (Tasking Supervisor/Staff for review and signoff) panel on the template to request secondary approval or attestation from another user. You can send a task to the approver requesting approval. The approver can use this panel to add their electronic signature and generate a Signature Attestation document that is separate from the template document. Child Welfare Demographics

You can document relevant demographic information such as contact information, address and employment history, emergency contacts, and family household information. You can use the CW Demographics (BH) template to enroll users in programs and capture demographic and other important information on participants in a child welfare program. With the Family Unit feature, you can group patients under a certain name or category. Each member of the household, including foster children, must be entered into a family unit. Each family unit must have a single member identified as the main participant. To update family unit information, select File > Family Unit > Unit setup. Child Welfare Phone Interview Panel The CW Phone Interview (BH) template enables you to document the initial screening details of prospective foster or adoptive parents. The details collected during this screening help in the subsequent interviews and encounters with foster parents. The panels on this template display the details documented on the CW Demographics (BH) and CW Home Interview (BH). Child Welfare Foster Dashboard

You can easily view empty beds available for foster children, and continue to track the locations of each foster child within the program. CW Foster Dashboard(BH) Panel The CW Foster Dashboard (BH) template enables you to view the beds that are available for foster children. You can view the locations of the foster children within the Foster Care or Adoption Service program. The data of foster parents and children are entered in the Bed Placement Configuration panel on the CW Demographics (BH) template. The assignment of foster children to a specific bed and foster family is done on the CW Demographic (BH) template of the foster family, main participant. Note: The data grid on the CW Foster Dashboard template gets the available bed details from the extended data grid in the Bed Placement Configuration panel on the CW Demographics (BH) template. Open the CW Training and Group Note Template

You can document details about the training and group sessions that foster or adoptive parents attend. In a group encounter, select the templates Template Icon icon. The Select Template window opens. In the Group tab, under Show, select All. The list of group templates appears. Select Template Panel Select the CW Training and Group Note (BH) template, and then select OK.
