Document Patient's Trauma History using the LEC-5 Interview Template
In NextGen® Enterprise EHR, in Patient History, select the Templates button. The Select Template window opens. On the Medical Records tab, select All. From the list, select LEC-5 Interview (BH), and then select OK. Select the program, specialty, and visit type for the patient. In the Rate Emotional Abuse Severity panel, enter the rating after completing all the questions on the patient's emotional status. In the Rate Physical Abuse Severity panel, enter the rating after completing all the questions on the patient's physical status. In the Rate Sexual Assault or Other Uncomfortable Sexual Experience Severity panel, enter the rating after completing all the questions on the patient's sexual assault experiences. In the Rate Other Stressful Event Severity panel, document the patient's exposure to other stressful events such as war, life-threatening illness, sudden violent death, or serious injury you have caused to someone else. To facilitate improved trauma care, the Trauma Hx Questionnaire (BH) template provides a general questionnaire for clients to self-report on stressful incidents and trauma in their lifetime. Clients can view and manage details related to serious or traumatic life events on the Trauma Hx Questionnaire (BH) template. In NextGen® Enterprise EHR, in Patient History, select the Templates button. The Select Template window opens. On the Medical Records tab, select All. From the list, select Trauma Hx Questionnaire (BH) and select OK. The Trauma Hx Questionnaire (BH) template opens. Expand the Serious or Traumatic Life Events panel. Serious or Traumatic Life Events Answer all the questions listed in the panel. Document the Patient's Mental Health and Risk Assessment Information
Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Document the Patient's Suicide Risk Assessment
Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Eval of Suicide Risk (BH), and then select OK. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. Document the reason for the suicide risk assessment. Document the patient's historical, clinical, and situational risk factors. Document the protective factors for the patient's mental health. Open NextGen® Enterprise EHR. For a patient encounter, open a template. Note: Your practice can also configure the template to be accessible from thePacket Navigation (BH)template. For more information, go toNextGen Healthcare Success Community, and download theConfiguration Guide forNextGen® Behavioral Health Suite. Select the Screening Tools link. The Screening Tools template opens. Select PSTD Checklist for DSM-5 (PCL-5). Select the assessment date. Select a severity score for each question. The total score is calculated based on the selected scores for all of the questions. Select Save & Close. The information appears in the Screening Tools Performed grid on the Screening Tool window.
