The purpose of a consent form is for the card holder to give the practice permission to store their credit card information as a unique token ID in the system. Any sensitive information such as the credit card number or the CVV number is not stored in the system.
- Forms Enhanced Template - You must use the Forms Enhanced template when creating the form.
- Electronic Signature - To save the form electronically on a patient's account, the practice must use an e-signature pad.
- Printing the form - After the card holder agrees to store their card data, by token ID, in the application you can print the form and give it to the card holder for their records.
