The purpose of a consent form is for the card holder to give the practice permission to store their credit card information in the system and with NextGen® Pay powered by InstaMed.
- Forms Enhanced Template- You must use the Forms Enhanced template when creating the form.
- Electronic Signature- Using a signature pad allows the patient to electronically sign the consent form
- Printing the form- After the card holder agrees to store their card data by token ID. Then you can print the consent form and give it to the card holder for their records.
