All the fields on correspond to the Field Locators on the UB claim form. For example, the occurrence code and date that is entered in the 32a field will populate the first line, which is considered line a, of Field Locator 32 on the UB claim form. Field 32b would correspond with the second line, which is line b, of Field Locator 32 on the UB claim.
Note: All entries must be added starting with 32A before 33A can be added.
The following example shows the top half of a UB04 claim form with lines a and b of the occurrence code fields.
The is used differently, depending on the type of user.
- If you are a Regular UB user, you can use the fields on this tab to manually enter the occurrence codes and dates that you want to print on the UB claim. Select the drop-down menu to display the list of occurrence codes.
- The occurrence codes and dates that are entered in these fields must be entered in the order that the fields display on the window. When one field is populated with an occurrence code and date, the field below it becomes enabled for you to enter the next occurrence code and date.
In addition, if you delete an occurrence code and date from a field, any populated field below it is also deleted. The field that you originally deleted will be the first enabled field that will allow the entry of an occurrence code and date.
- If you are a Recurring Billing UB user and you want to use the automated UB claims process, these fields are automatically populated by the application and should not be changed.
- The occurrence codes and dates that are automatically entered in these fields are entered in the order that the fields display on the screen. These occurrence codes are automatically carried over to subsequent encounters. Any occurrence code that is changed or manually entered is not carried over. If you delete an occurrence code and date from a field, any field below it that was populated will also be deleted. The occurrence code fields that were changed are no longer automated.
- The occurrence codes that populate these fields are linked to the revenue codes that are attached to the service items.
- The revenue codes that are used for the automated UB claim process and have the appropriate linked occurrence codes are 420 – 429, 430 – 439, and 440 – 449.
- The occurrence codes that are linked to the revenue codes for automation carry over to subsequent encounters; including occurrence codes 35, 44, and 45.
- Occurrence code 11 is an automated occurrence code and is carried over to subsequent encounters.
