Consolidate Payments and Diagnoses Codes

Consolidate all previous payer payments, adjustment data, and diagnosis codes for COB claims. Line-level payments and adjustments are maintained at the maintained at the line level. Claim-level payments and adjustments are maintained at the claim level in the Other payer loop. Associated data is consolidated at the claim and line level. Encounter-Level Diagnosis Codes

Diagnosis codes are typically entered during charge posting. Charge-level diagnosis codes then display in the ICD-CM section on the General sub-tab on the Encounters tab in the Patient Chart. After charge posting, you can add additional encounter-level diagnosis codes, if needed. You can also add encounter-level diagnosis codes prior to charge posting, so they default to each charge during charge posting. Note: Diagnosis codes added to an appointment from the Ins/Diag tab in the Add/Modify Appointment window automatically default to the ICD-CM section on the General sub-tab in the Patient Chart > Encounters tab when the appointment is checked-in and the encounter is created. Add a Diagnosis Code for Diagnosis

Click OK on the alert message. Select the procedure billing details in the grid. The data appears in the corresponding fields above the grid. Do one of the following: Click the Dx Description 1 and select the diagnosis code from the list. Click Cancel on the list. The diagnosis search window opens. Search and select the required diagnosis. The selected diagnosis and the associated code appear in the Dx Description 1 and Code fields respectively. If applicable, repeat the above steps for Dx Description 2 through Dx Description 4 to add additional diagnosis. Note: The diagnosis selected in the Dx Description fields uploads in the Today's Assessments grid on the Add or Update Assessment template. Click Update. If you add additional data in the grid, the Verify Charges button appears in the Procedure Billing panel. Apply Different Diagnoses to CPT Codes

If needed for a claim, each CPT Code in the Charges grid can have a different Dx Code attached. Select the CPT Codes in the Charges grid. When a charge is selected, a check mark appears in the first column for that charge line and the buttons below the grid become active. Select the Remove DX button to remove all diagnoses from the selected CPT Codes. In the Diagnosis grid, select the diagnoses that you want to apply to the selected charges. Select the diagnoses in the order of priority. The diagnosis listed as 1 will be the primary diagnosis assigned to the CPT codes. Do any of the following: To apply the selected diagnosis to the selected CPT Code(s), select Apply DX. To apply the selected diagnosis to all CPT Code(s), select Apply DX to All. Add an Existing Diagnosis to the Current Encounter

Open NextGen® Enterprise EHR. For a patient encounter, open an assessment template. From the Assessment/Diagnosis panel, select a diagnosis in the Active Diagnosis grid. The Diagnostic description and ICD Code field fills in. Alternatively, in the Last Encounter Diagnosis for Current Provider section, select Copy Forward. Note: When all the diagnoses from the last encounter for the current provider are active, the data is forwarded to the Diagnosis grid. If one or more such diagnosis is not active, the Copy Forward button is not available. You can select the diagnoses manually to add them to the Diagnosis grid. Add Status, Severity, Differential Dx, and Impression. To enter specifiers in the Specifiers field, do one of the following: Select in the field and enter free text. Select the My Phrases link and select a phrase from the list. In the Present on Admission field, select a description from the list. The Description field fills in based on the selection made. Sort Treating Diagnoses

The Treating Dx grid show diagnoses in the order that they will appear on the claims form. You can sort the order of the items in the Treating Dx grid, to put the most relevant items for today’s encounter at the top. Select Sort DX. The NTS Diag Sort window opens. The top grid shows all diagnosis in their current order. NTS Diag Sort template displaying existing diagnoses sort order Select the primary diagnosis for today in the top grid. The diagnosis moves to the lower grid, as #1 in the new order. Select the additional diagnoses in the order of their relevance to the treatment for that visit. Diagnoses move to the bottom grid in the order they are selected. Select Save & Close. The window closes. The Treating Dx grid shows the new order.
