Providers Practice Tab

If the provider is a rendering physician for your practice, then select the Rendering Provider at this Practice check box. The check box enables the remaining fields in the Practice Provider Information section.

Rendering provider at this practiceCheck this box to add practice-specific information for this provider. The provider information entered here only applies to the practice in which you are working. If the provider is a rendering provider at more than one practice, you must enter information for each practice by changing the service location in the Service Location field.

Selecting this check box also allows the provider to be assigned to a user during user setup in the System Administrator application.

Supervising provider at this practiceSelect to make this provider a supervisor for the practice in which you are working. This setting works in conjunction with Supervisor Billing.
Signature on FileSelect this check box when you have the rendering provider's signature on file. This will populate the CLM06 element position of the ANSI 837 Electronic sendfile. The Signature on File setting initially defaults to activated for all existing providers in your system and when you create a new Provider Master record and check the Rendering provider at this practice check box.
Supervisor RequiredFor Incident-To billing, select this check box to require a supervisor for an encounter when the provider is the rendering provider. DEA NumberEnter the provider's DEA number, which is used to track controlled substances from the time they are manufactured until the time they are dispensed to the patient.
NADEA NumberEnter the provider's Narcotics Addiction DEA Number. The number usually begins with the letter "X" and indicates that the provider is allowed to prescribe certain medications that treat narcotic addiction. State License NumberEnter the State License number for the provider.
Tax ID Number/ SSNEnter the Tax ID number that the provider will use for this practice or, if no Tax ID exists, the Social Security number. This will display in box 25 of the HCFA 1500.
Provider TypeSelect a Provider Type for the provider. This may be used for reporting purposes in NextGen® Enterprise PM. It may also be used in the configuration of the Claim Modifiers library to add modifiers on claims for charges that are rendered by specific types of providers.
SpecialtySelect a Specialty for the provider. This may be used for reporting purposes in NextGen® Enterprise PM. It may also be used in the configuration of the following for additional functionality:
  • Anesthesia library
  • Consolidated Services Claim library
  • Claim Modifiers library
  • Insurance Authorizations in NextGen® Enterprise PM
PDMP Gateway RoleSelect a PDMP (Prescription Drug Monitoring Program) provider role type from the list of PDMP roles displayed.

Role types defined in this list, which are approved by Appriss Health, would be able to request for and view a patient's PDMP report.

Zone 1, Zone 2, Zone 3Select the geographical areas used in NextGen® Enterprise EHR to assign to the provider. The zones in the list are set up in the Master Lists.
Enable Supervisor BillingWhen selected for a mid-level rendering provider, claims can be created in the name of a supervising provider and enables you to perform supervisor billing at the provider level.
Bill as Non-PersonWhen a provider bills under the facility name, select this check box. For more information on non-person billing, refer to the Claims Guide for NextGen® Enterprise PM.
Service LocationSelect the service location where the provider is rendering. The list only contains locations attached to the practice that you are logged into.

A service location is a place for receiving services that has the same tax ID number as the practice. You can then add group information for the service location. If the provider is rendering at more than one location, then group information will have to be entered for each location.

Contract SubgroupingSelect the contracts that the provider participates in.
Group InformationThe information that displays in the columns in the Group Information section only displays if there is more than one service location for the practice. When a new provider is created, a default group information record is included here.
