Globally Update Pricing in a SIM Library

You can globally update the pricing of service items in a SIM library. You can filter these SIMs based on a SIM or CPT4 code range, effective date, and department. The global update enables you to change the SIM pricing based on a percentage, a specific dollar amount, or a geographically-adjusted RVU. The adjustments can be rounded up or down.

In File Maintenance, select Libraries > Service Items.
Right-click the applicable library, and then select SIM Global Update
.

The SIM Global Update window opens.
You must enter an Effective Date to determine which SIMs are updated. SIMs are updated only if this date falls between the SIM effective and expiration dates.
To further specify which SIMs are updated, you can apply additional filters by:
By doing one of the following:
- Enter a range of SIM codes to update the SIM codes within this range.
- Enter a range of CPT4 codes to filter for CPT4 codes in this range.
- Select a department to filter for SIMs associated with it. A department can be used with any combination of other filter criteria.
If you want to use a dollar amount or a percentage to calculate the pricing, select Use Dollar/Percentage under New Price Using Dollars / Percentage. Do anyone of these:
- Select the Dollar option to update the fees by a flat dollar amount, and then enter the dollar amount in the Amount Increase field.
- Select the Percent option to update the fees by a percentage, and then enter the percentage amount in the Percent Increase field.
If you want to use RVU and GPCI information to calculate the price, select Calculate Price using RVU*GPCI under New Price Properties Using RVUs and do the following:
1. Select the RVU library.
2. Select a GPCI library, if necessary, and then select a carrier or locality.
3. If you want to use the RVU library to calculate the SIM RVU, select an RVU in the RVU field (this list of RVUs comes from the RVU library), and then the GPCI value in the GPCI field. You can select up to three RVUs and GPCIs to use in the calculation. Each RVU is multiplied by a GPCI and the results are added together. For example: (RVU1 x GPCI1) + (RVU2 x GCPI2) = SIM RVU
  
  Note: If you select zero ( 0) as the RVU or GPCI value, the result of the multiplication is zero. If you select 1, the result is equal to the other factor.
4. For Pricing Multiplier, enter the adjustment percentage for the price so that correct payment for the service can be made. If the price of a service in a particular area is high, you can use the multiplier to adjust the price. For example, a multiplier of 100% results in a flat Medicare rate while a multiplier of 115% increases the price by a multiple of 1.15.
5. Enter the Conversion Factor or, if the price is anesthesia-related, enter the Anesthesia Conversion Factor. The conversion factor is the Medicare conversion factor that the RVU and GPCI calculation (geographically adjusted RVU) is multiplied by to get a dollar payment amount. For example, in 2006 the conversion factor was 36.177. For the remaining conversion factor field, you must enter 1. You must enter a value in both conversion factors fields. For the remaining conversion factor field, you must enter 1. You must enter a value in both conversion factor fields.
Select which kind of prices you want updated for each code by selecting either Update the Non-Facility Price or Update the Facility Price option.

Note: To update facility and non-facility prices, run the update twice – once for facility and once for non-facility.
To create a new row of prices for each code, select Create New Effective SIM Price, and then enter the new date range Effective Date and Expiration Date fields in the New Price Properties section. OR To update pricing in existing rows, select Update Existing SIM Effective Price. Existing SIM prices will be updated even if there is a future iteration of range prices for that SIM.
If you want to prevent existing prices from being lowered during the update, select Do Not Reduce the Price.
Select an option in the Rounding Option field to determine how the updated prices are rounded. For more information, see SIM Global Update Rounding Options.
Select OK.
The SIM Global Update List window opens. Selected check boxes indicate the SIM records that will be updated. If you selected the Do Not Reduce the Price option, SIMs that have new prices that are lower than the old prices have cleared check boxes. SIMs with clear check boxes are not updated.
If you do not want certain SIMs to be updated, then you need to clear the check boxes for those SIMs.
Select one of the following:
- Update – A message confirms that you want to create new prices for the selected SIMs. Select Yes to update the pricing and generate a report of the changes or select No to cancel the pricing update process.
- Report – A message warns that the update process will cancel if you run the report. Select Yes to run the report or select No to cancel the report.
The Service Item/CPT4 Global Update report opens.

In File Maintenance, select Libraries > Service Items.
Right-click the applicable library, and then select SIM Global Update
.

The SIM Global Update window opens.
