Add an Allergy Reaction and Severity

You can document an allergy reaction and severity for the allergy. Reaction-severity details appear wherever allergies display. Although allergy reactions and severities are SNOMED® codified, you can create user-defined reactions but not user-defined severities. Contact your system administrator for more information. From the Medication Allergies module, in the Reactions section, select Add. The Add Reaction (Severity) window opens. Reactions marked as common list first. Begin typing the name of a reaction or scroll through the list of reactions, and then select a reaction. Unknown is located at the top of the Common Reactions list. Specify a severity of the reaction. Select Add. The allergy reaction and severity appear in the Reactions section. You can add up to five allergy reaction and severity. Allergies - User Defined

The Allergies master file contains a list of user-defined allergies. From the Master Files - System list, select Allergies. The Allergies - User Defined List window opens. Do one of the following: To add a new item, select open menu , and then select New. To modify an existing item, select the item in the list, select open menu , and then select Open. You can also double-click to open the item. The Add/Modify Allergy Information window opens. Enter the abbreviation for an allergy in the Allergy Key field. This field allows for a maximum of 8 characters. Enter the full description of the allergy in the Allergy Description field. Enter any additional information for the allergy in the Note field. If you want to hide the item in the list, select the Hide check box. Select Next to add another item or select OK to return to the Allergies - User Defined List window. Add a Medication Allergy to a Patient Chart

Open NextGen® Enterprise EHR. Open the Medication Allergies module for the selected patient. If the patient has any existing allergies, select Clear. The values in all the fields are cleared. Note: Clearing the fields prevents you from editing an existing allergy. To search and filter an allergy, do the following: Select the search icon next to Allergy. The NextGen - Allergy Selection window opens. Allergy Selection Select one of the following filters from the Filter list: Contains Searches for allergies that contain with the search criteria. Ends With Searches for allergies that end with the search criteria. Ingredient Searches from the list of drug ingredients, such as Iodine or Bentonite. Medication Name searches from the list of medication allergies, such as cefazolin with codeine. Allergen Group Searches from the list of allergens, such as lithium derivatives or ester type anesthetics. View Patient's Medication Allergies

Before you begin You must ensure that you have the View access right enabled in MODULES > Medical Records > Medications Allergies in System Administrator. For more information on user access rights, go to NextGen Healthcare Success Community, and download the latest System Administrator Guide forNextGen®Enterprise. You can view a quick list of the patient's active medication allergies to identify potential adverse drug reactions. You can also open the Medication Allergies module to add or update the patient's allergies. A total count of the patient's active allergies appears on the Allergies button on the Quick Buttons panel. Note: The icon indicates that the patient account does not have any active allergies records and the No Known Allergies check box in Medication Allergies is not selected. Diagnosis Categories Search

From the Master Files - System list, select Diagnosis Categories. The Diagnosis Categories List window opens. To add a new diagnosis category, select Open Menu , then select New. To search for an existing diagnosis category to modify, follow the steps below: In the List Search field, enter what you want to search for. The matching results are listed. Note: You can enter an asterisk ( * ) to see all the records. Double-click the diagnosis category you want to view. The Add/Modify Diagnosis Category Information window opens. Enter a name for the diagnosis category in the Category field. Enter a four-digit abbreviation for the diagnosis category in the Short Description field. To add an ICD code and description select Open Folder . The ICD Codes window opens. To add a code to the category, select the code in the Available list, then select the Include . The code moves from the Available list to the Included list. If necessary, select either 09 or 10 in the Show field to display only ICD9

Patient Allergy Trigger Overview

You can run the Patient Allergy trigger to add an allergy directly to the patient's allergy list instead of using the Medication Allergies module. The trigger action updates the module. The Patient Allergy trigger is set up in Template Editor. Note: If you execute this trigger to add an allergy that already exists, a message appears, asking if you want to add the allergy again. You can select Yes to add the allergy again, or select No to cancel the action. The trigger may be set up to add multiple reactions for an allergy, up to a maximum of five. You may also be able to manually enter a reaction in a data entry field. The system locates and stores any SNOMED codes associated with allergy reactions. If a Drug Utilization Review (DUR) is set up, you can check for interactions between each patient allergy that is added and all of the active medications prescribed to the patient.
