Forms not sent to NextGen EHR

You can use the Search Patient Form section to locate forms that are not sent to NextGen® Enterprise EHR.
  1. Open NextGen® PxP Portal Practice Portal.
  2. Select Custom Forms
    The Search Patient Forms section appears by default.

  3. To define the scope of your search, use one or more of the following filters.
    • Select the status of the form. You can search for All, Open, Viewed, Printed, or Completed forms.
    • Select the practice's location.
    • Select the type of form. You can search for various types of published, unpublished, and deleted patient forms.
    • Enter the patient's first name, last name, or date of birth.
    • Select the date range for the search. You can search for a maximum of three months of completed forms at a time.
  4. Select Search.
    All patient forms matching the search criteria display.
