Past History

You can enter Past History information by selecting any of the links in the left navigation for Function, Nutrition, Pediatric, Lymph/Oncology, Cognition, and Fluency. Past History grid When you have opened any Past History template, you can use the left navigation to jump to another template. Note: To fill in these templates more efficiently, you can use Get Data to pull information from prior visits, or QuickPicks to enter saved favorites. See Use Get Data and QuickPicks for more information. After you have entered all information on a template, select Save & Close. All information is stored in the Past History grid. Document and Review Patient Pain for a Physical or Occupational Therapy Visit

You can document the patient's pain levels for the current day, when they are at rest, and when they are active. You can also review the pain history within the case to assess the patient's pain levels over time. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Single Visit template, and then expand the Subjective panel. Enter the pain levels in Today, At Rest, and During Activity. Optional: To review the patient's pain levels over time within the case, select the Pain link. The Pain History template opens with the pain details of all the encounters related to the case. To document the pain experienced during the patient's daily functions, select Function in Subjective Grid to open the Function template. Optional: To view historical pain levels from the Function template, select the Previous Pain Grid link. The Previous Pain Grid template opens with pain data from previous versions. Document a Speech Therapy Health Promotion Plan

You can document screening and follow-up plans of care for the following: BMI, depression, and hypertension health promotion plans Pain management and fall risk plans of care Functional status Note: You can customize the Health Promotion Plan template in NextGen® Adaptive Content Engine. For more information, go to the NextGen Healthcare Success Community and download the latest NextGen® Adaptive Content Engine Health Promotion Plan Quick Reference Guide. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Speech Single Visit template associated with the encounter, and then expand the Plan panel. Document Caregiver Education for Speech Therapy

You can document the speech therapy training the patient's caregiver receives in the patient's absence. The patient or representative must have given consent for this training. The caregiver training information appears on the Office Visit document. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template associated with the encounter, and then select an SLP visit type. Open the NTS Speech Single Visit template, and then expand the Objective panel. Under Treatment, select Education. The Education template opens in the Speech Education window. Select Patient consented to caregiver training. Select the appropriate checkbox that describes whether the caregiver training is for an individual or a group. For individual caregiver training, enter the total minutes of training time in Min, and then enter any therapist assistant-led training time in Asst. Document Patient Education for Speech Therapy

Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template associated with the encounter, and then select an SLP visit type. Open the NTS Speech Single Visit template, and then expand the Objective panel. Under Treatment, select Education. The Speech Education template opens. Under Education, select a field to open a list of education descriptions, and then do one of the following: Select a description from the list to insert it into the field. Select Free Text to return to the field, and then enter a custom description of the education provided. Optional: Repeat the previous step to document additional patient education. In Patient and/or Caregiver Education, enter additional information related to the patient education or caregiver training. This information appears on the Plan of Care and Office Visit documents. Select Save & Close. The SLP Education row is added to the Treatment grid in the Objective panel. Parent top

Single Visit Subjective Panel

Use the Subjective panel on the NTS Single Visit template to document the patient’s history. The Subjective panel also gives you access to additional templates to document the patient’s limitations, work demands, and specialty subjective information. In the Job Title, Work Status, and Last Worked fields, you can document the patient’s workplace and work status information. Note: These fields share the same values from the Job Title, Work Status, and Last Worked fields on the NTS Homepage template. The Ordering Diagnosis grid pulls data entered on the NTS Homepage template. To update Ordering Diagnosis information, open the NTS Homepage template. The Surgery and Past Medical History grids display any history that was captured in a NextGen Healthcare application for the patient.
