You can document screening and follow-up plans of care for the following: BMI, depression, and hypertension health promotion plans Pain management and fall risk plans of care Functional status Note: You can customize the Health Promotion Plan template in NextGen® Adaptive Content Engine. For more information, go to the NextGen Healthcare Success Community and download the latest NextGen® Adaptive Content Engine Health Promotion Plan Quick Reference Guide. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Single Visit template associated with the encounter, and then expand the Plan panel. The Health Promotion Plan template is used to document screening and a follow-up plans of care for Quality Reporting Care Plans, which are required for successful Quality Reporting. The template includes the following types of Quality Reporting Care Plans: BMI Health Promotion Plan Depression Health Promotion Plan Hypertension Health Promotion Plan The template also includes additional panels for documenting the screening and follow-up plan of care for pain management, fall risk, and functional status for successful Quality Reporting. You can document a patient's health promotion plan on the Health Promotion Plan template. You can document the following details on the Health Promotion Plan template. BMI plan Depression plan Hypertension plan Pain management plan Fall risk Screening tools and follow-up care plans Functional and Cognitive Assessment Status Health Promotion Plan You can select the information icon to open the Information Display window, which displays all the details required for successful quality reporting. For adult patients, you can document the diet, physical activity, and referral details. For patients younger than 18 years, you must document the diet and physical activity details. The Exceptions link allows you to manage the exceptions on the Exceptions/Measures template. This link appears in all panels except Functional and Cognitive Assessment Status. Add a BMI (Weight Management) Plan

You can use the Health Promotion Plan panel to enter a BMI Plan, Hypertension plan, Depression plan or a customized health promotion plan. Your selection in the Plan field determines the other fields that appear on the panel. For more information, go to NextGen Healthcare Success Community, and download the Family Practice User Guide for NextGen® Adaptive Content Engine. Expand the Health Promotion Plan panel. Select BMI Plan from the list in the Plan field. Click the Diagnosis field to search for and enter a diagnosis. Enter recommendations for Diet, Physical Activity and Referrals in the respective fields. Click Add. The plan details are added to the grid. When you are finished entering BMI data, do one of the following: Click OK to close the window. Select another type of plan in the Plan field and complete the entries for that plan.
