Surgical History of Patients

You can document a patient's surgical history and orthopedic surgical history. You can use the following sections on the Intake template to document and view the surgical procedures performed for the patient. Orthopedic Surgical History. You can document and view the orthopedic-specific procedures performed for the patient. Surgical History. You can document and view all other surgical procedures performed for the patient. These sections are shared with several templates of the NextGen® Orthopedic Suite, and the grids in these sections display the date of surgery, global date, side, and description. The grids are sorted by DOS column, and the date of surgery can be entered in the MM/DD/YYYY, MM/YYYY, or YYYY formats. To enter a global date, the date of surgery must be in the MM/DD/YYYY format. Add Medical History

You can add medical history such as diseases, disorders, and surgical procedures of a patient. On the appropriate template, in the Medical History section, click Add. The Past Medical History template opens. Past Medical History template Note: By default, the Specialty field displays Orthopedics. In the Medical and Surgical panels, do the following steps for the options without a link. Select the desired check box. The details Details icon icon and a blank date field appear. Click the date field. The Date Selection window opens. Click the calendar icon, and select the date when the disease or disorder first appeared or when the surgery is performed for the patient. Note: You can also enter the date in the field. To add the documented details to the grid in the Past Medical History panel, click Add To Grid. Add Medical and Surgical History

You can add the medical and surgical history of a patient on the Histories template or the History Summary panel. You can add medical and surgical history information in the History Summary section. If there is no relevant history information for a patient, you can select No relevant past medical/surgical history. Open NextGen® Enterprise EHR. Open the Intake template. In the History Summary section, select Past Medical/Surgical. Select Add Past Medical/Surgical Hx. Document the reported history information in the corresponding section. Select Add To Grid, and then select OK.The history records appear in the Past Medical History and Past Surgical History grids. Add Additional Past Medical History You can document additional past medical history on the Past Medical History template. Categories in History Summary

You can document various history details for a patient in different categories in the History Summary panel. The left pane of History Summary panel on the Intake template includes a list of categories to select and view the documented data. The following table provides a description of each category. Category Description Past Medical/Surgical Enables you to document the patient's past medical and surgical history. You can document the patient history using Add. If no medical or surgical history exists, you can select the No relevant past medical/surgical history checkbox. You cannot select the checkbox if medical surgical history present in the grid. Interim Enables you to document the interim history. Social Enables you to document the Social history. You can select an option from the left pane using Add/Edit. Remove a Medical Condition

In the Past Medical History panel, you can remove a medical condition from the grid. Access the Configure Medical, Surgical, Family Hx system template. Expand the Past Medical History panel. Select Specialty and select a specialty from the list.The grid displays the medical conditions configured for the selected specialty. To view medical conditions configured for a specific practice, select Practice and select the practice from the list. Select a Filter above the grid to filter data in the grid.The grid displays medical conditions associated with the selected filter condition. For more information on how to filter conditions in the Past Medical History panel, see Filter the Medical Conditions. Modify the Medical Conditions

In the Past Medical History panel, you can update the medical conditions in the grid. Access the Configure Medical, Surgical, Family Hx system template. Expand the Past Medical History panel. Select Specialty and select a specialty from the list.The grid displays the medical conditions configured for the selected specialty. To view medical conditions configured for a specific practice, select Practice and select the practice from the list. Select a Filter above the grid to filter data in the grid.The grid displays medical conditions associated with the selected filter condition. For more information on how to filter conditions in the Past Medical History panel, see Filter the Medical Conditions. Highlight a condition in the grid to be modified. The data populate in the fields above the grid. Make relevant edits in the applicable fields, and then select Update.The updated conditions appear in the grid.
