Surgical History of Patients

You can document a patient's surgical history and orthopedic surgical history. You can use the following sections on the Intake template to document and view the surgical procedures performed for the patient. Orthopedic Surgical History. You can document and view the orthopedic-specific procedures performed for the patient. Surgical History. You can document and view all other surgical procedures performed for the patient. These sections are shared with several templates of the NextGen® Orthopedic Suite, and the grids in these sections display the date of surgery, global date, side, and description. The grids are sorted by DOS column, and the date of surgery can be entered in the MM/DD/YYYY, MM/YYYY, or YYYY formats. To enter a global date, the date of surgery must be in the MM/DD/YYYY format. Diagnosis Details of Patients

The Diagnosis grid displays the orthopedic diagnoses or general medical diagnoses of the patient, depending on the selected option above the grid. The Diagnosis grid displays data entered on the applicable templates of the NextGen® Adaptive Content Engine and NextGen® Orthopedic Suite. You can also add, edit, and remove diagnoses of the patient. Diagnosis grid Add the Patient's Diagnoses Edit a Diagnosis You can edit the description of a diagnosis. Add Medical History

You can add medical history such as diseases, disorders, and surgical procedures of a patient. On the appropriate template, in the Medical History section, click Add. The Past Medical History template opens. Past Medical History template Note: By default, the Specialty field displays Orthopedics. In the Medical and Surgical panels, do the following steps for the options without a link. Select the desired check box. The details Details icon icon and a blank date field appear. Click the date field. The Date Selection window opens. Click the calendar icon, and select the date when the disease or disorder first appeared or when the surgery is performed for the patient. Note: You can also enter the date in the field. To add the documented details to the grid in the Past Medical History panel, click Add To Grid. Past Medical and Surgical History

The Past Medical History grid displays any Medical and Surgical history that was captured in a NextGen Healthcare application in your organization or practice. For example, if you have one practice but multiple locations, all locations can see the Past Medical History regardless of where it was entered. Manage Surgical History of Patients

On the appropriate template, in the Surgical History or Orthopedic Surgical History section, select the procedure to be edited or removed from the grid. Surgical History grids To edit the selected procedure, do the following: Click Edit below the corresponding grid. The IORTS Px Edit Detail window opens. IORTS PX Edit Detail window Make the desired changes. Note: The CPT4 field is read-only. The Global Date field is automatically filled based on the surgery date for the procedures documented in the patient history sections on the Surgery Coding, Single Fx, and Multi Fx templates. Click Update. The updated details appear in the grid. To remove the selected procedure, click Remove below the corresponding grid, and then select Added in Error from the list. The procedure is removed from the grid. Note: You must be on the encounter where the procedure is added, to remove the selected procedure. Add Medical History

Select the Medical HX button below the grid. The Past Medical History template opens. Select the checkbox next to the history you want to add. Items in black prompt for an onset date. Items with blue labels open a picklist to select from, to further specify the PMH for that item. For example, if you select Cancer, a picklist opens to select the type of cancer. Select the type of cancer, and then select OK, where upon you are prompted to enter the onset date. If you select Other, a search window opens. Here you can search for the specific PMH.
