Documenting Pregnancy Details

On the Pregnancy Outcomes template, you can document the pregnancy details of a patient.

The following table describes the fields in the Detailed Entry/Edit panel on the Pregnancy Outcomes template.

Pregnancy Entry TypeYou can select the entry type of pregnancy.
Pregnancy #You can select the order of birth from the list.
Multiple gestation birth orderIf multiple gestation is applicable, you can select the order of birth.
SexYou can select the option for gender of the child.
StatusTo indicate the status of the child at birth and present.
Provider nameYou can select the provider on the Physician window.
Place of deliveryYou can select the hospital from the list.
Month, Day, and YearIn the corresponding fields, you can enter the month, day, and year. You can also click the calendar icon, and select the date on the Calendar window.
TimeYou can select the time format, and enter the time in the selected format.
Labor (hrs)You can enter the total labor hours for delivery, and enter the time separately for first, second, and third stages.
Baby nameTo enter the name of the baby.
Supporting person(s)You can enter the name of one or more of the supporting persons.
Gestational ageTo indicate the type of the delivery. You can enter the gestational age in weeks and days.
Birth weightYou can enter the weight of the baby in pounds and ounces. The equivalent weight in grams is automatically filled in the Gm field.
APGARIn the 1 min, 5 min, and 10 min fields, you can enter the corresponding APGAR scores.
LacerationsTo indicate the degree of laceration.
EpisiotomyTo indicate the type of episiotomy.
OtherYou can select the type of laceration in the list. You can also add another type of laceration in the subsequent blank field.
Delivery TypeYou can enter the details in one of the following sections, as needed.
  • ≥ 20 weeks
  • < 20 weeks
≥ 20 weeksIn this section, you can select the type of delivery, and add the following details.
  • Postpartum Tubal Ligation
  • Labor induction
  • Analgesia/anesthesia
Postpartum Tubal LigationIf tubal ligation is performed after the delivery, you can select this check box.
Labor inductionYou can select the appropriate options for labor induction from the list.
Analgesia/anesthesiaYou can select the options for anesthesia from the list.
< 20 weeksIn this section, you can select the type of miscarriage, and add management type.
ManagementYou can select the type of termination management from the list.
DetailsYou can select the appropriate options for the history of pregnancy, and type any additional comments in the blank field.
