Physical Exams (Technicians/Assistants)

The exam links listed in the Physical Exam panel depend on whether the panel is on the Prelim Exam - OPH template or the Provider - OPH template. On the Prelim Exam - OPH template (seen as the Prelim Exam tab), the Physical Exam panel includes only exams performed by the technician or assistant, which are later reviewed and updated by the provider. On the Provider - OPH template (seen as the Provider tab), the Physical Exam panel includes additional exams that are only performed by the provider. The Physical Exam panel on the Prelim Exam - OPH template. Documented exam findings appear in separate grids for the right eye OD and the left eye OS. Abnormal findings are indicated with an exclamation mark ! in the grids. The Refresh button causes new or updated exam findings entered on the panel to appear in the grids. Low Vision Exam

The provider can document the exam for a low vision visit on the LV Exam - OPT template. You can review the testing performed by the technician and perform additional exams as needed. Enter Visual Acuity Findings Document an External Exam You can document the external exam of an eye in the External Exam panel. Document a Slit Lamp Exam Document a Posterior Exam Enter Cover Test Findings Enter Nystagmus Findings You can record findings associated with nystagmus in the Nystagmus panel. Record Color Vision Findings You can record color vision deficiencies in the Color Vision panel. Document Corneal Light Reflex Test Results You can enter your findings from the Corneal Light Reflex test in the Corneal Light Reflex Test panel. Gonioscopy Exam

The detailed Gonioscopy exam is optional. Gonioscopy exam findings can be viewed or documented on the Gonioscopy – OPH template, which can be opened from three different places: Slit Lamp Exam Detail template - during technician/assistant and provider workflow Provider – OPH template – on the Physical Exam panel during provider workflow OPH Physical Eye Exam template – during provider workflow Note: When you make an entry on the Gonioscopy - OPH template causes Gonioscopy to appear in the list of billable procedures on the Finalize - OPH template. Document a Slit Lamp Exam

You can document a slit lamp exam in the following areas: In the Physical Exam panel, go to the Slit Lamp section. slit lamp exam panel For a more detailed exam, click the Slit Lamp link. Previosuly documented findings appear in the current enocunter fields. To copy the last exam, select the Copy last exam check box. To indicate that the exam findings are within normal limits, select the Slit Lamp WNL check box. Update the exam findings in the corresponding fields. To save the documented values as the default values, click the Quick Save, and enter a name for the template. This feature enables you to use the same values while documenting the same template during future encounters. To load the previously documented default values into the current template, click the Quick Load . On the Provider - OPH template, the Physical Exam panel includes: Exams performed by the technician or assistant, which are later to be reviewed and updated by the provider Exams that are performed only by the provider Providers can update exams using the following options to document in the Physical Exam panel: Exam links in the left pane of the OD and OS grids Other links in the panel Exam fields Abnormal exam findings appear in the grids with an exclamation mark and a red circle appears in the left pane next to the corresponding exam. You can use the Refresh button to update the grids after you document or update exam findings. Ophthalmology Drawings

Ophthalmic providers can create drawings to document exam findings. Links to the ophthalmic images appear at the bottom of the Provider - OPH template or can be accessed by clicking the Draw link. After the provider draws the exam findings, they are saved into the patient's chart and appear in the History Toolbar. The following images are available in the Ophthalmology specialty: Draw AC Draw Amsler Grid Draw Cornea Draw CVF Draw External Draw Face Draw Gaze Positions Draw Gonio Draw Lacrimal Draw Lens Draw Macula Draw Optic Nerve Draw Periphery Draw Pupil Draw Vitreous Draw Custom Draw Blank Page You can access the drawings in the Visual Acuity panel by clicking the Draw link. On the retina procedure templates, you can click the Draw button. The Draw button and link opens a list of different drawing backgrounds for Ophthalmology.
