Set Up the Practice as an ASC Provider in the Provider System Master File

The Ophthalmology ASC practice is required to be set up as an ASC Provider in the Providers system master file. Before beginning to enter patient information for Ophthalmology ASC, you must select your ASC Center in the top toolbar as the Provider.

  1. Click Providers under Master Files - System to open the Providers List window.
  2. Click the Menu icon below the Providers List Search field and select New ... to open the Add Provider Information tabs.
  3. On the Demographics tab, enter name, address, and display name for the provider (fields in red font are required).

  4. Select the System tab and enter the following information:
    • UPIN
    • Taxonomy code
    • NPI
    • Default Tax ID
    • Provider Licensing

  5. Select the Practice tab and do the following:
    1. Select the Rendering provider at this practice check box.
    2. Select the Signature on file check box.
    3. Select the Bill as Non Person check box to set the 2100B NM102 value to “2” instead of “1” on electronic claims.
    4. Enter Group Information - Double-click into the <default> Payer row and enter the following, then click OK:
      • Provider Number (Tax ID #)
      • Effective and Expiration Dates
      • Group Name
  6. Select the Provider Types tab and verify that all surgeons listed as a provider can be selected as a Referring Provider on encounters for the ASC facility.
  7. Click OK when you have completed entering information for the practice as a provider.
  8. Click Close to close the Locations Search window.
    The ASC Center as a provider displays in the Provider List.
