Document Patient's Current Tobacco Use Details

You can view and document the current tobacco use details for a patient.

  1. Open NextGen® Enterprise EHR.
  2. To open the Social History – Tobacco template, do one of the following:
    • On the Histories template, on the Social panel, select Tobacco/Vaping Use, and select Add/Edit.
    • In the History Summary panel on Intake, select Tobacco/Vaping Use from Social, and select Add/Edit.
    The Social History – Tobacco template opens.
  3. Expand the Tobacco Use panel.
  4. For Have you ever used tobacco?, select No/never, Yes, or Unknown.
    • If you select No/Never, then continue with Document Passive Exposure.
    • If you select Yes, do one of the following:
    • Select Yes or No for Have you used tobacco in the last 30 days?
    • Select Yes or No for Have you used smokeless tobacco products in the last 30 days?
  5. Under the Smoking Tobacco Use and Non-Smoking Tobacco Use sections, select the applicable Tobacco type for the patient.
  6. To record daily tobacco use, select Use daily.
  7. Enter a value in the Usage per day field.
  8. Enter the details in Years used, Pack year, Age started, and Age stopped fields.
  9. If the entries that appear automatically in the Smoking status or Tobacco use status fields do not accurately reflect the patient's situation, select the fields and select a more accurate description from the list.
  10. If there is no change in the selected options in the Tobacco Use panel from the previous encounter, select the Reviewed check box to indicate that the provider has reviewed the details.
    If you select a different option in the current encounter from that of the previous encounter, then Reviewed check box is selected automatically.
