On the
Histories
tab, select a row in the
Past Ocular History
grid and click
Remove
.
The highlighted row no longer appears in the
Past Ocular History
grid.

Update Existing Ocular History
You can update previously documented ocular history. On the Histories tab, select a row in the Past Ocular History grid and click Edit. The Ocular History – OPH template opens and displays details for the highlighted row. Make any applicable changes to the information displayed in the fields. Click Save and then click Close. Indicate the Type of Ocular History Review
Before you begin Review patient’s past ocular history. Select the appropriate check box to indicate that the patient’s past ocular history. Select Reviewed, no changes if there is no new ocular history. Select Reviewed, updated when you add or revise the ocular history. Parent topic: Past Ocular History

Document Ocular History
You can document new ocular history on the Ocular History - OPH template. On the Histories tab, click the Add in the Past Ocular panel to open the Ocular History – OPH template. ocular history - oph Select an ocular category. The category determines the specific lists of choices for disease and procedure or management that open. Enter the following details, as needed: Disease - the associated SNOMED code automatically fills the corresponding field Eye Date Dx Procedure - the associated SNOMED code automatically fills the corresponding field Eye - eye involved in the procedure Global end Date – refers to the global time period for post-operative days included as part of the billing for the procedure; this is the last date of the global time period and based on the selected procedure date and the global time period defined by the practice Proc Date - procedure date Surgeon Location Outcome Click Save. Past Ocular History
You can add, update, and view a history of ocular conditions, procedures, and treatments for the patient on the Past Ocular History panel. You can use the fields above the grid to document that the patient has a prosthetic eye and to indicate the type of review performed. Remove Ocular History Data
You can remove an OPH Ocular record from the Ocular History Setup template to remove a SNOMED codified ocular disease or procedure. In the Custom DB Picklists template top grid, select OPH OcularHistory. Click Configure. The Ocular History Setup template opens with pre-configured data in the grid.
