Working with Closed Loop Referrals

To help you track referrals effectively and meet patient needs, you can use NextGen® Enterprise EHR 8 to send referrals using the 360 Exchange Closed Loop Referral (360x) profile. NextGen® Enterprise EHR8 supports the 360 Exchange Closed Loop Referral (360x) profile of the Patient Care Coordination (PCC) domain, as defined by IHE International. Note: Because closed loop referrals are not fully supported by most Electronic Health Records (EHR) vendors, NextGen Healthcare is introducing closed loop referrals through a pilot program. If you want to be able to initiate closed loop referrals, contact NextGen Healthcare Support and ask to be enrolled in the pilot. Closed loop referrals enable the initiating provider and receiving provider to send each other status updates. Each provider can see the referral status and quickly view the actions that are available and whether a response is pending from the other provider. Prerequisites for Closed Loop Referrals

For your practice to use closed loop referrals, NextGen Healthcare Support must configure the following: Install or upgrade to NextGen® Enterprise EHR 8. Configure NextGen® Share or a Direct message destination. Define providers in HIE Gateway as referral recipients with at least inbound access. Ensure that each provider is assigned a unique National Provider Identifier (NPI) issued by the National Plan and Provider Enumeration System (NPPES) at https://nppes.cms.hhs.gov. You can assign the NPI for a provider in File Maintenance under Master Files > System > Providers > <provider name> > System > National Provider ID. To ensure that referrals reach the correct provider, you must use an official NPI value that is not reused for different providers. Viewing Closed Loop Referral Requests

In the Data Share module, on the Closed Loop Referrals tab, the Patient's Referrals list displays inbound and outbound referrals for the currently loaded patient. Only 360X-compliant (closed loop) referrals are shown. Initially, the list includes only referrals to or from the current provider (the provider selected in the Top Toolbar). However, you can change the settings in Advanced Parameters to include referrals to or from all providers. Closed Loop Referrals The following columns are available for each referral request in the Patient's Referrals list. Column Heading Description Create Date The date that the referral request was first initiated, not the date of the most recent action for that request. Next Step Buttons for any available actions. Different buttons are listed, depending on the current status of the referral request and the directionality. Send Updated Information for a Closed Loop Referral

The 360 Exchange Closed Loop Referral (360x) profile does not provide a way for the initiating provider to send updated clinical data to the receiving provider. To fulfill this need, NextGen® Enterprise EHR sends such updates as direct messages instead of closed loop messages. In the Data Share module, select the Export tab. On the Encounter History tab, select a newer encounter than the one used for the original referral request. This newer encounter can include newer clinical information, such as test results. In the To field, specify the original referral recipient. In the Associated Referral field, select the same referral order that you did in the original referral request. If you need to add an attachment, then do the following: In the Clinical Information pane, set Export Type to Manual Data Selection. In the Attachments pane, select an attachment. Fill in the remaining fields the same way that you did in the original referral request, then select Preview and Export. NextGen® En

Managing Outbound Closed Loop Referrals

When you initiate a referral in the Data Share module, it is sent as either a simple referral or a closed loop referral, depending on the capabilities of the receiving system. For a closed loop referral, you can use the Closed Loop Referrals tab to view or cancel the referral. You can also use the Export tab to send updated information or to send the referral to a different provider. Cancel Closed Loop Referral Requests You can cancel a closed loop referral if you decide that it is no longer needed. Change the Receiving Provider for a Closed Loop Referral While a closed loop referral request is in progress, you can create a new referral request to send the same referral order to a different receiving provider.
